Two things to know about endo: Ablation is NOT the gold standard. Excision is. When should my child have plastic surgery? This is a great question that comes up frequently in my practice, and the answer is not simple. I treat newborns for congenital ear deformities; 5- and 6-year-olds for misshapen or prominent ears; 12- and 13-year-olds for overweight and obesity; and 15- and 16-year-olds for cosmetic and functional nose concerns. I carefully consider physical age and emotional and intellectual maturity in the timing of treatments, and think that decisions of whether to treat and when to treat should be carefully considered by all parties involved. Ideally, a child is able to understand the details of treatment, and the risks and benefits and recovery involved, but of course this is not always possible. In the setting of newborn congenital ear correction, I must start treatment ideally within the first 2-3 weeks of life, and recommend treatment be started as early as possible for best chance of success. In this setting, I make treatment recommendations to parents and related healthcare providers like Pediatricians and Audiologists. In the setting of cosmetic and/ or functional rhinoplasty, I like to have a thorough discussion with the patient and caregivers before recommending treatment. The patient's autonomy, the drive to do no harm, the desire to improve functional concerns and to take away causes for insecurity--I carefully consider all these factors during my decision-making and treatment. #plasticsurgery #medicalethics #child #medicalcare #aesthetics #reconstructivesurgery #rhinoplasty #otoplasty #nosesurgery #nosejob

Arthrofibrosis is a condition where too much scar tissue forms in and around the knee after a total knee replacement (TKR), causing stiffness and pain: 1. Inflammation and Healing Response: After knee surgery, the body reacts to the trauma by sending signals to heal the area. This involves releasing chemicals called cytokines that help form a blood clot and attract immune cells (white blood cells) to the site. 2. Excessive Scar Tissue Formation: - Chemicals like PDGF and TGF-β promote the growth of cells that produce collagen and other materials, which build up and create thick scar tissue. - VEGF is another chemical that increases inflammation and scar tissue by promoting new blood vessel growth in the knee. 3. Lack of daily high intensity stretching: With home exercises only being 10% of the torque of a physiotherapist’s manual therapy the knee isn't moved enough after surgery. So tissues become stiff, and the excessive production of collagen leads to continuation of scar tissue build up. This causes a loss of ROM, impacting simple daily activities.
