IT’S NOT WHAT YOU THINK By Richard J. Wolff, RDN Forty years ago, Exercise Scientists challenged the status quo by suggesting older adults engage in high-intensity Strength Training. Until then, Strength Training Programs often used light household objects like soup cans. While there's nothing wrong with soup cans, they aren't heavy enough to strengthen muscles, leaving Scientists to believe that Strength Training didn't work for older adults. However, research on aging from Bill Evans, Ph.D., Chief of the Human Physiology Laboratory at Tufts University, set the stage for a new era in Strength Training. In BIOMARKERS, Ten Keys to Prolonging Vitality, Dr. Evans writes, "One of the most enduring myths of aging is that as we grow older, we lose the capacity to enlarge and strengthen our muscles. This myth has been aided and abetted by research studies designed by scientists with low expectations of their older test subjects. These researchers did not push their older subjects. Instead, they had them Strength Training at minimal intensities. Today, Dr. Evans is a Professor of Medicine in the Division of Geriatrics at Duke University Medical Center. Given that it's been 30 years since BIOMARKERS was published, I contacted Dr. Evans and asked if he still recommends high-intensity Strength Training for older adults. He responded by saying, "As you know – I recommend the use of high-intensity Strength Training for older adults that could result in muscle failure." I'm reading through a first responder paper right now. The first thought that comes to mind is that, to me, fitness after aerobic adaptation is a better predictor of occupational fitness (and subsequent CVD risk) than the test itself. This is to say, administrators should evaluate one's ability to be aerobically fit enough to perform the job (in this instance, LE) after a period of aerobic training. Little clip documenting my SCI clients progression with his back extension ability. Prior to performing these with no external support from equipment or myself, we performed these for months holding weight in one hand, and using the other hand to hold onto his chair, assisting him to extend his back and sit back upright which contributed towards building strength in this variation whilst relying on grip for stabilisation. My client previously had to use his hands & arms to press himself back to sitting upright….. Not any more! With programming cycles, I like to implement a combination of compound lift modifications, isolation work and dynamic sequences to keep my clients training varied, diverse and nonlinear, with specific outcomes and goals in mind, this way my clients don't get bored easily and their journey doesn't feel too one dimensional. Ideally, I aim to make the exercise selection complimentary with previous or ongoing physiotherapy work, I don't like the idea or have the intention of my clients going into their sessions feeling as though they are commiting to another clinical appointment… on top of appointments! or repeating any therapy work. As the rehabilitation training goes on, I plan on gradually shifting our focus from addressing areas for improvement less, to capitalising on the clients potential and how we can continually move forward, although complex cases usually involve barriers and hurdles along the way, which is important to acknowledge and understand whilst learning the clients abilities and needs, I think its beneficial not to have every session with troubleshooting as a sole focus to avoid discouragement and instead, help view their goal posts with physical exercise more positively. #rehabilitation #clientprogress #gym #exercise #coach #rehabpersonaltrainer #westyorkshire #SCI #spinalcordinjury #scitraining #functionalmovement #mobility #stability #balance #coordination #bodyweightexercise
