A retrospective cohort study at 29 academic medical centers involving over 2,400 hospitalized adults revealed that 23% experienced diagnostic errors, with 17.8% causing temporary harm, permanent harm, or death. The study, led by Dr. Andrew Auerbach of the University of California San Francisco, highlighted patient assessment, test ordering, and interpretation as major factors in diagnostic errors. Diagnostic errors were implicated in 6.6% of cases resulting in death. The findings suggest higher-than-expected error rates and associated harms, prompting a need for improved diagnostic processes. The study, based on the Vizient Clinical Data Base, acknowledged limitations such as bias and incomplete data but underscores the significance of addressing diagnostic errors in healthcare. 🌞 Saturday morning reflection: I was fully inspired by this article. Accountable care organizations and value-based care have been hot topics for over ten years now with some additions and twists along the way. I thought about how lab medicine would fit in in this healthcare model countless of times but never put my thoughts into such organized, actionable buckets as mentioned in the article. 💡"Think of yourself in the laboratory as first responders. We’re the first to know and to be able to identify something at a population level that can then be translated to an individual." So insightful and thought provoking. The Laboratory collects and maintains so much health data that could be used to create strategies and care plans to address unmet needs in the population served. The demonstrative clinical lab 2.0 projects aim to create such models. Looking forward to reading their upcoming published project outcomes.

I am thrilled that the first paper of my dissertation has been published! We report on the development and validation of the Treatment-Preference-Measure-Advance Care Planning (Treat-Me-ACP). An early-stage outcome instrument that uses hypothetical scenarios to measure the dynamics of patient preferences and complements the evaluation of the effectiveness of advance care planning interventions. Termed as continuity of care, this model not only enhances patient health but also alleviates the workload for family doctors. Key Findings: 🔍 Improved Patient Health: The study indicates that maintaining a long-term relationship with a single doctor enhances patient health outcomes significantly. 📉 Reduced Doctor Workload: Contrary to common assumptions, the research reveals that having a consistent GP actually reduces the workload for family doctors. According to the study, patients who consistently saw the same GP experienced an 18% longer interval between visits compared to those who consulted different doctors. This finding challenges traditional perceptions and underscores the value of continuity of care in modern healthcare systems. The implications of this study are profound, emphasizing the importance of fostering lasting doctor-patient relationships.
