CMS Introduces New Billable Codes for Caregiver Training Services: A Step Forward with Significant Challenges Ahead The Centers for Medicare & Medicaid Services (CMS) has introduced new billable codes for caregiver training services, effective in 2024. This initiative is a commendable first step in recognizing and supporting family caregivers of Medicare beneficiaries. However, the road ahead is fraught with significant challenges that need to be addressed to ensure this policy achieves its intended impact. The Promise and the Pitfalls The introduction of these new codes means that physician fee services can now support family caregivers in a more structured manner. This is a welcome development for the many caregivers who provide essential support to their loved ones. However, several critical questions and potential issues need to be addressed: Time Allocation Concerns How does this new code account for the expanded time needed in an appointment to discuss caregiver training? - Current appointment durations haven't magically increased. Physicians are already pressed for time, and adding caregiver training discussions into the mix without extending appointment times could be challenging. Integration into Existing Services - How will it be integrated into the exam, or does a patient and their caregiver need to know to ask for it? - Clarity is needed on whether these services will be proactively offered or if caregivers must explicitly request them. Many caregivers might not be aware of their availability or feel comfortable asking. Accessibility and Implementation When, where, and how is it accessed by the caregiver? How do healthcare systems adopt and implement these new codes? Are physicians even aware of this change? These questions highlight the need for a clear, systemic approach to implementing these services. Training and communication within healthcare systems are crucial to ensure all stakeholders are informed and equipped to utilize these codes. #TheMajorRedFlag: #TelehealthExclusion 🚩 These services cannot be accessed via telehealth at this time. They are only available in person, and the Medicare member must be present unless consent is granted. This is a significant problem. Caregivers often juggle multiple responsibilities and may not have the time or ability to attend in-person training sessions. Telehealth could provide a more flexible and accessible solution, especially for those with demanding schedules. The House Subcommittee on Health recently advanced the Telehealth Modernization Act of 2024 (H.R. 7623), aiming to extend key Medicare telehealth flexibilities through 2026. Here’s what’s on the table: 👉 𝐄𝐱𝐭𝐞𝐧𝐝𝐞𝐝 𝐀𝐜𝐜𝐞𝐬𝐬 𝐚𝐧𝐝 𝐑𝐞𝐢𝐦𝐛𝐮𝐫𝐬𝐞𝐦𝐞𝐧𝐭 The bill proposes to continue the hospital-at-home program until 2029 and remove geographic restrictions on telehealth visits through 2026. 👉 𝐄𝐦𝐩𝐨𝐰𝐞𝐫𝐢𝐧𝐠 𝐇𝐞𝐚𝐥𝐭𝐡𝐜𝐚𝐫𝐞 𝐏𝐫𝐨𝐯𝐢𝐝𝐞𝐫𝐬 If passed, the bill would allow the HHS Secretary to expand the types of professionals eligible to provide reimbursable telehealth services. 👉 𝐅𝐨𝐜𝐮𝐬 𝐨𝐧 𝐑𝐮𝐫𝐚𝐥 𝐇𝐞𝐚𝐥𝐭𝐡 Special attention is given to rural health clinics and Federally Qualified Health Centers, enabling them to serve as distant sites for telehealth. 💡 𝐖𝐡𝐚𝐭’𝐬 𝐍𝐞𝐱𝐭? While the bill has cleared the Subcommittee, it must still pass in both the House and Senate. But these are not the only changes around telehealth legislation. Several states are passing new bills, extending virtual care in their states. 🔹Alaska's SB 91, if voted for, will enhance telehealth provisions by allowing out-of-state members of a physician’s multidisciplinary care team to offer services via telehealth. 🔹Over in Iowa, the Board of Nursing is proposing new licensure requirements for certified professional midwives, emphasizing standards for telehealth services. 🔹New Jersey’s A2803, if passed, will extend authorization for certain out-of-state health care practitioners and recent graduates of health care training programs to practice in the state. 🔹SB 403 in New Hampshire has recently passed the first chamber and is set to authorize certified community health workers to deliver services through telemedicine, broadening access and enhancing community health efforts. Healthcare organizations----- If you're ready to train your team on the value of CUSTOMER SERVICE, I'm here for you!!! When we first begin and I'm doing my "Discovery", these are common themes: 1. your professional caregiver team on their phones 2. your professional caregiver team with ear buds in, ignoring residents/patients who are calling out for help or in clear need of help IF your caregiver did not have her face in her phone (I have personally preventing SEVERAL falls because I intervened when residents needed help; I could not get the caregiver's attention) 3. Ignoring resident/patients who are walking without their walkers 4. Dementia residents out in the courtyard (they can come/go as they please) but one resident with a walker was unable to manage her walker plus try to open the courtyard door. A resident VISITOR got up to help the resident back in because the caregiver was oblivious. And when she noticed the VISITOR helping the resident, it never dawned on the caregiver to intervene, help, and thank the visitor for doing what the caregiver should have been doing 5. Caregivers wheeling residents backwards in their wheelchairs 6. Med techs approaching dementia residents from behind to give them their meds and then wonder why the resident "refuses" their meds---after the resident has been startled and becomes agitated (this happens more often than you realize--and then the resident goes without their meds) 7. Receptionists on their phones and not greeting visitors as they enter the building (that's kind of their job) 8. Nurses in rehab centers running around frantically because they have 8 admissions that day---and how that looks to family members who are already petrified that they had to move their loved one into a community. 9. OFTEN, I overhear families/visitors talking to themselves about how they don't see anyone in leadership making rounds or providing much supervision or management, especially when they are the ones getting up to help residents vs the staff intervening 10. Smiling and just even saying "hello" when families are visiting or touring If you wondered if your CUSTOMERS (the patients/their families) notice these things, YES THEY DO.
