Why is a patient scrambling for the maintenance prescriptions because these are not being processed when the specialist fails to inform the general practitioner about the handover for maintenance prescriptions? More specifically, a patient treated by the cardiologist for low ejection fraction is prescribed Entresto and Carvedilol with the patient being told that they will be on these permanently. The cardiologist, after a year or two and establishing that patient is responding well determines no need for follow-up appointments. BUT, the patient is not informed that the prescriptions are now the general practitioner‘s responsibility for maintaining refills! The inevitable result is that the patient is left without a refill and is only then informed of the lack of communication between cardiologist and general practitioner. Further, the general practitioner has no current record of the cardiologist handover! Advice to other patients: Buck up cowboys!
