Take a look at the picture Rob has here. 4 total joints. The unwrapped trays were used. 23 total. The wrapped trays weren't used. 26 total. The unwrapped trays will be unwrapped and all 49 (by my unofficial count) will be put on a courier van... Taken back to Regional Offices. Checked in. Inspected, inventoried and stored. Then 2-3 days later... Put back on a truck, delivered to the EXACT SAME ASC FOR THE SAME SURGEON AND REP... For cases the following week. Because ASCs typically don't have room to store all of the instruments required for all of their cases. If you are old enough, and listen VERY CLOSELY... You can hear the Benny Hill Theme Song in the background. When I say Instrument Processing is a laborious slog of futility, this would be an example. Thanks Rob McClaskey. Surgeons: Why should you care about sterilization? You are incentivized to care about the costs of implants, anesthesia, and other surgical processes via bundles, HQEP or as shareholders. Sterilization and associated waste are significant issues in surgery that are virtually unaddressed. The national average cost to sterilize 1 tray is $57.87 (Goldberg et al,JOA#34,2019). Pictured are 50+ vendor trays a device rep transported from hospital to ASC for 4 total joints. 25+ backup trays were unwrapped, costing thousands in waste. They will all be sterilized and returned to the hospital in 48 hours where the weekly cycle will begin again. This happens everyday in every hospital and ASC. Vendors cannot supply more trays. The only solution is to standardize efficiency. Pods lower and fix sterilization costs while decreasing OR turnover time and labor. In this example, using Pods would have cut over 66% of the sterilization cost while providing other important efficiencies. One actionable step for most ophthalmologists is to take a look at your surgical pack! Many standardized ophthalmological surgery packs come with items that are not used - this requires them to be cleaned again or thrown away if disposable. It is best practice for surgeons to collaborate with their nursing and surgical tech staff to create a pack in which each item is intentionally chosen for its specific use. Some specific things you can do today: -change from a full-body drape to a face drape only -ensure that the cover for your instrument stand is the smallest size that is still feasible -have a huddle with your nursing and surgical tech staff to create your perfect pack!
