2023 was a year of strong momentum for VillageReach as we race towards an ambitious goal of reducing inequities in access to primary health care for 350M people by 2030. We didn’t just maintain pace; we aimed higher, strategically expanding proven solutions across new and familiar geographies. We did so with a renowned purpose and focus on responsive primary health care – to dismantle the inequities and barriers that prevent access to quality care for under-reached communities. Achieving ambitious goals is a marathon, not a sprint. But here’s the secret: we’re not running this race alone. This year’s Annual Impact Report, titled “Running the Race, Together,” emphasizes the power of radical collaboration with governments, the private sector, and public health champions. 💚 Green social prescribing and Active Travel social prescribing. Do you know what it is? 🤔 Well you can find out more here... We're a part of Active Travel England's, Active Travel Social Prescibing Pilot in a several areas across the UK including Bath and North East Somerset. 👟 Active travel social prescribing involves recommending or prescribing physical activities such as walking or cycling as a means of transportation to improve health and well-being. It's instead of relying solely on traditional modes of transportation like cars or public transport, individuals are encouraged to incorporate active travel daily routines. This approach promotes physical activity, reduces carbon emissions, and can lead to various health benefits such as improved cardiovascular health, weight management, and mental well-being. It's a holistic approach that integrates physical activity into everyday life, enhancing both personal health and environmental sustainability. 🌿 Green social prescribing involves the recommendation or referral of individuals to engage with nature-based activities or environments to improve their physical, mental, and social well-being. It acknowledges the therapeutic benefits of nature and encourages people to connect with the outdoors for health benefits. ❤️ This patient wellbeing approach helps to: improve mental health outcomes, reduce health inequalities and reduce demand on the health and social care system 💫 The Active Way with Bath & North East Somerset Council project starts with 26 hyper-local walking routes across communities like Midsomer Norton, Paulton, Radstock and more. We're workind to ensure that everyone can participate in the joy of walking, regardless of ability or background. Almost half of our trails have been designed to be flat without steps or stiles, inviting individuals of all ages and fitness levels to put on their shoes and join us on a new journey of exploration. Many of the routes start and end at community GP surgeries and healthcare facilities. Change happens when we come together. 🪧 Today, civil society groups representing health, social justice, disability, environment, cycling and active travel came together calling on Dublin City Council to fully implement the Dublin City Centre Transport Plan without delay, fostering more active travel, walking and cycling. I had the honour of representing the Irish Heart Foundation highlighting: ***Problem*** 🔴 80% premature heart disease and stroke is preventable through healthy lifestyles including physical activity. 🔴 The health system will never be able to cope with demand given the growing and aging population. 🔴 There are profound health benefits to physical activity but less than half of people in Ireland meet national targets. 🔴 When government talks of chronic disease prevention, too often it means awareness campaigns. 🔴 We must call out the impact of harmful vested interests used to delay adoption of health promoting policies. There is a disproportionate focus on telling people to change their 'unhealthy lifestyle' instead of system-level change to support more active lives. How can people have choice when the odds are stacked against them? ***Solutions*** ✅️ We need policies that stop preventable chronic diseases happening in the first place. ✅️ The Dublin City Centre Transport Plan is a prime opportunity to create a positive shift in supporting Dubliners to be more active. ✅️ Dublin City Council's own Healthy Ireland plan commits to optimising "opportunities where people can be supported and empowered...to ensure they achieve their fullest health potential". ✅️ Health is a political choice. The Irish Heart Foundation calls on the Dublin City Council CEO to fully implement the Dublin City Centre Transport Plan without delay.

We are pleased to share with you Manifest MedEx’s 2023 Annual Report, which highlights our strategic priorities, key achievements, the volume and quality of health data exchange that MX enables, and the impact the data we exchange make in and across CA communities.     Thanks to our partnership with our network participants, regional and state public health agencies, and local community stakeholders, today Manifest MedEx is a crucial part of the state’s health data infrastructure, securely bringing permitted, critical health data to participants across every county to improve care, advance health equity, and support CalAIM and public health priorities to help those most in need.     In 2023, more than 240 organizations joined our network. We delivered an average of 1.6 million hospital admission, discharge, and transfer notifications and almost 6 million CCDAs per month — providing robust real-time information needed to coordinate care and improve outcomes. We invested in ways to bring higher quality and more meaningful data to our participants and individuals — all while doing our part to protect the privacy and security of Californians' sensitive health data.     As health data exchange continues to evolve, we remain steadfast in our commitment to building the digital health data safety net needed to support our participants, those outside our network, and California more broadly.  Read our report: https://lnkd.in/gHinBpJs

🌟 Honored to Share Insights from Becker's 14th Annual Conference! 🌟 Last week in Chicago, I had the privilege of attending Becker's 14th Annual Conference, where I had the pleasure of hearing from Mike Slubowski, the esteemed President and CEO at Trinity Health. 🎤 In his enlightening keynote address, Mike underscored the critical importance of prioritizing **People, Process, Technology, and Culture** in the ever-evolving healthcare landscape. His strategic emphasis on these pillars serves as a guiding beacon for industry leaders navigating complex challenges. 💡 🤝 Of particular note was Mike's astute recognition of the pivotal role that **Partnerships** play in addressing the pressing needs of our communities. By fostering collaborative relationships, we can leverage collective strengths to drive meaningful impact and innovation. 🏥💫 Additionally, Mike's candid critique of the term "value-based care" provided a refreshing perspective. He aptly highlighted the need to move beyond mere cost considerations and focus on delivering quality care while ensuring equitable **Access** to essential services. However, what truly resonated was Mike's profound belief in the sanctity of healthcare: "We’re about people caring for people at the end of the day. I believe that it’s sacred work." 🙌✨ This poignant reminder reinforces our collective dedication to serving with compassion and integrity. 💪🌟 As we reflect on Mike's insights, let's remain steadfast in our commitment to excellence and patient-centered care.
