RADIOSURGERY for INTERMEDIATE GRADE (III) or HIGH GRADE (IV-V) AVMs (Spetzler-Martin): International Stereotactic Radiosurgery Society (ISRS) PRACTICE GUIDELINES Graffeo et al., Neurosurgery, July 2024 https://lnkd.in/dXrkxQTr 1634 AVMs (1431 Grade III, 186 Grade IV, and 11 Grade V lesions) treated with RADIOSURGERY - the crude OBLITERATION RATE for Grade III AVMs was 72% - the crude OBLITERATION RATE among Grade IV-V AVMs was 46% - post-SRS HEMORRHAGE was observed in 7% of Grade III compared with 17% of Grade IV-V lesions - major PERMANENT DEFICITS or DEATH from hemorrhage or radiation-induced complications occurred in 6% of Grade III and 12% of Grade IV-V AVMs CONCLUSION: Most patients with Spetzler-Martin Grade III AVMs have FAVORABLE RADIOSURGERY treatment outcomes; however, the OBLITERATION rate for Grade IV-V AVMs is LESS than 50%. "REPEAT SINGLE-SESSION RADIOSURGERY for CEREBRAL ARTERIOVENOUS MALFORMATIONS (AVMs): A Systematic Review, Meta-Analysis, and International Stereotactic Radiosurgery Society (ISRS) PRACTICE GUIDELINES" Shaaban et al., Neurosurgery, June 2024 https://lnkd.in/dZXcg6Zh 14 studies with a total of 925 patients that underwent repeat single-session RADIOSURGERY (SRS) for cerebral arteriovenous malformations (AVMs) - median FOLLOW-UP was 21-50 months - at the time of repeat SRS the Spetzler–Martin grade of the AVMs was: *grade I for 12.54% of patients *grade II AVMs for 46.51% of patients *grade III AVMs for 34.92% of patients *grade IV for 5.08% of patients *grade V for 0.95% of patients - the MEDIAN prescription DOSE was 15-25 Gy - the MEAN prescription DOSE was 13.06-22.8 Gy - the pooled overall OBLITERATION RATE was 59% - the pooled HEMORRHAGE INCIDENCE was 5% - the pooled overall RADIATION-INDUCED CHANGE incidence was 12% CONCLUSION: For an INCOMPLETELY obliterated AVM, REAPEAT RADIOSURGERY after 3 to 5 years of follow-up from the first SRS provides a REASONABLE BENEFIT to the risk profile. After repeat SRS, OBLITERATION is achieved in the MAJORITY of patients.
