The NICE-NHS move in implementing subscription-based model for antibiotic evaluation and payments, is a significant shift in health policy. By promoting subscription contract based payments, this model likely aims to address the challenges of antibiotic resistance as well as incentivise pharmaceutical companies to develop and provide antibiotics that promotes responsible use and stewardship.

QI change is hard to sustain. Kudos to IHQSE - Institute for Healthcare Quality, Safety & Efficiency graduates from an antimicrobial stewardship team that have now sustained their ‘handshake stewardship’ program for over 13 years! This model allows provider autonomy to order any antibiotic without pre-approval. The team rounds daily and has discussions with providers about their cases, assisting in their decision making.  Consistent with a massive decrease in broad spectrum antibiotic use the team found significant reductions in C. difficile diarrhea and pharmacy savings of over $2 million/year. And providers love the service the team provides!
