Yesterday, the Senior Care Pharmacy Coalition (SCPC) and healthcare analysis firm ATI Advisory released a report consisting of evidence supporting the capabilities of LTC pharmacy to reduce cost of care and improve outcomes for seniors in home and community-based settings. Grane Rx is pleased to be included in this report, where our Meds2Home program is cited as an example of the comprehensive med management support that LTC pharmacy can provide seniors outside of an institutional care facility. “Aging in place is an ideal - but understandably challenging - context for delivering care. Eligible adults age 55+ who participate in PACE, the Program of All-Inclusive Care for the Elderly—a combined Medicare and Medicaid program—are able to access a comprehensive range of services, including medical and dental care, physical therapy, prescription drugs, and long-term services and supports. PACE has demonstrated success in improving the health and well-being of participants. It has high satisfaction among family caregivers and is cost effective. Still, many states don’t have the program. https://lnkd.in/gqbzDpZB In light of this horrific story my heart goes out to this family and the pain they have and will continue to endure! When will our government see that we need clinicians who assist aged care facilities and community run hospice and palliative care organisations, on the proper procedures for admitting palliated clients? Such admission requirements as: ensuring driver orders are current and available, medication orders are current, treating doctors are actually willing to prescribe opioid medication and symptom relief medication, and strict procedures for pressure area care are adhered to! No person should die in pain and pressure areas should not be happening. Again, the system has let their client down. I propose that our government hire educated clinicians in palliative care to attend each and every aged care facility to audit their palliative care practices, educate the nursing team and ensure compliance. This CANNOT be controlled by the aged care facility as it has to be impartial and fair. Further to auditing the facilities, a push towards looking at community palliative care admissions and revamping their systems should be included. We have Work Safe in our country, why can’t this too be a government run program to ensure the safety and health of our palliative clients?

This is one of the things that drives me crazy about our industry- Acting like LTC/Custodial are the same as SNF/Post Acute. They’re simply not. I truly believe the biggest reform our industry needs is differentiating between the two just like we did ALF/RCF/SNF’s. Primarily long term facilities attract a completely different type of employee a niched post acute attracts. The financial comparison is also no comparison at all. I don’t know why we are so confused why the blanket policies and reform aren’t working when we keep trying to make them fit two wildly different models. Separate. Them.
