The six dimensions of The ASAM Criteria were updated in the Fourth Edition. The Third Edition’s Dimension 4: Readiness to Change does not contribute independently to the recommended level of care; rather, it impacts clinical judgments related to risks in other dimensions and influences the services that should be delivered at any level of care and should be carefully considered in treatment planning. The Fourth Edition reorders the dimensions such that consideration of readiness to change is integrated across dimensions and replaced by a new dimension, Dimension 6: Person-Centered Considerations. This new dimension considers barriers to care—including social determinants of health (SDOH)—patient preferences, and the need for motivational enhancement. The Fourth Edition also simplifies and updates dimension names to reflect the field’s evolving terminology and facilitate effective dissemination of these standards. Meet Dr. Kathryn A. Cunningham, Director of the UTMB Health Center for Addiction Sciences and Therapeutics at The University of Texas Medical Branch. Substance use disorders (SUDs) pose a significant challenge, as they often damage brain function related to motivation and decision-making. While psychedelics like psilocybin have shown promise in helping with recovery, their risks—such as hallucinations and safety concerns—make them challenging to use widely. Dr. Cunningham is working on a solution that focuses on developing new compounds that similarly boost brain function but without harmful side effects. This approach could open up new, safer treatment options for those struggling with cocaine and stimulant addiction, areas where no approved medications currently exist. “Treatment plan, and Treatment plan’s engagement plan”. Over years, psychiatric treatment settings appear more inclined towards symptom focused therapy, with strong treatment plans taking a backseat. A comprehensive and early treatment plan is foremost in chronic psychiatric condition management, with stakeholder involvement pivotal in treatment success. Moreover, a cogent treatment plan requires a superimposed treatment plan’s engagement plan. The engagement plan needs continuous steering to keep the treatment plan on improvement pathway relative to medical and social aspects of centered individual.
