🧠 New Insights into Nerve Block Safety: Fascicular Injury from Needle Puncture is Rare 🧠 In the field of regional anesthesia, concerns about peripheral nerve injuries are always at the forefront. A recent study led by Victor Varela and his team sheds light on the risk of fascicular injury during needle transfixion in median and ulnar nerves. Key Findings: 🔍Out of 400 needle punctures conducted on isolated human nerves, only 3 instances of fascicular injury were detected. 🔍All injuries were caused by 15° beveled needles, but the low incidence suggests that needle trauma alone is unlikely to cause significant post-block neurological dysfunction. Implications: 📊 This study emphasizes that while needle puncture rarely results in fascicular damage, the complexity of nerve injury mechanisms means we must continue to exercise caution during procedures. 💡 As healthcare professionals, staying informed about such findings helps us enhance patient safety and improve outcomes in our practice. For the first time, federal health officials recommended this week that physicians counsel women about pain management before the procedure to insert an intrauterine device (IUD). Faculty Beverly Gray, MD, director of Duke Ob/Gyn’s new Complex Family Planning fellowship, notes in The New York Times, “Women’s pain and women’s experiences have been downplayed throughout medicine. Adenoidectomy is the surgical removal of the adenoid for reasons which include impaired breathing through the nose, chronic infections, or recurrent earaches. The effectiveness of removing the adenoids in children to improve recurrent nasal symptoms and/or nasal obstruction has not been well studied. The surgery is less commonly performed in adults in whom the adenoid is much smaller and less active than it is in children. It is most often done on an outpatient basis under general anesthesia. Post-operative pain is generally minimal and reduced by icy or cold foods. The procedure is often combined with tonsillectomy (this combination is usually called an "adenotonsillectomy" or "T&A"), for which the recovery time is an estimated 10–14 days, sometimes longer, mostly dependent on age. A recent study by Sjöcrona et al. (JEO Journal, 2024) reveals that Chronic Exertional Compartment Syndrome (CECS) of the forearm affects a broader range of patients than previously thought. Notably, 21% of patients diagnosed with #CECS in the forearm had a history of the same diagnosis in the lower legs. The study's findings challenge the current diagnostic criteria, suggesting that the widely used 30mmHg intramuscular pressure (IMP) cut-off may be too high for forearm compartments. Key occupations affected: office workers (21%) and craftspeople (18%). Common physical activities: strength training (21%) and cycling (15%).
