A picture is worth a thousand words. Many states still receive very low scores on the Leapfrog patient safety ratings. Why do we allow healthcare facilities to operate despite these low patient safety scores? Hospitals with a patient safety score of C or lower have an over 80% higher fatality rate due to preventable medical errors. These hospitals are endangering patients, while our regulators seem to look away. "The Leapfrog Group recently contracted with the Johns Hopkins Armstrong Institute for Patient Safety and Quality to update its estimate of deaths due to errors, accidents, injuries and infections at “A”, “B”, “C”, “D” and “F” hospitals. Overall, an estimated 160,000 lives are lost annually from the avoidable medical errors that are accounted for in the Leapfrog Hospital Safety Grade, a significant improvement from 2016, when researchers estimated 205,000 avoidable deaths. The analysis also found that D and F hospitals carry nearly twice the risk of mortality of A hospitals. The Leapfrog Group grades are out. The LFG Hospital Safety Grade is the only hospital rating focused exclusively on hospital safety. 'A' grade hospitals proudly hang lobby banners and celebrate. A few 'B' hospitals do the same. What do the 'C' grade hospitals do? Poorly led hospitals with toxic and abusive 'leadership' force good people out the door. Patient safety grades may falter with high turnover and transient staff. Is it any wonder frogs are limp or dying when care teams are sparse?
