Unlocking the Power of Patient Engagement in Orthopedics 🚀 Hinge Health recently published a study (https://lnkd.in/gVMcJaDz) that, as Orthopedics This Week points out, unsurprisingly, shows that “patients with OA that participated in a digital MSK program to manage OA have lower rates of total joint arthroplasty in the 12 months after enrollment.” Now, we all know insurers love finding ways to kick the expensive surgery can down the road, and this study plays right into that strategy. And the industry is already in violent agreement about the benefits of exercise to alleviate some of the symptoms of osteoarthritis. But it is one thing knowing you should exercise, and entirely another thing to do it. So, does the key to success really relate to effective patient engagement rather than the digital MSK program itself? Too often, patients expect surgery to be a magic fix, preferring to be passengers in the whole process, but when they take an active role in their own well-being, it’s a game-changer, benefiting both the individuals and the payors. The Hinge Health digital program, although offering only 3 to 8 different exercises, is a step in the right direction. That’s probably 3-8 exercises that they would probably not be doing otherwise. It’s not clear if the control group participants were left entirely to their own devices, but anecdotal evidence suggests that regular in-person or digital nudges really encourage compliance. Gamification of the PT process even more so. What’s more, while the duration of the programs (seemingly around 12 weeks) may seem short, it takes an average of 66 days to form a new habit, so maybe (just maybe) the app is kickstarting a more proactive approach to health. The real excitement lies in the future. These apps are getting better and better, evolving to offer more and more mass customization. This means information and activities increasingly tailored to each patient, potentially helping them delay surgery and guiding them through pre-hab, weight loss, and the entire joint replacement process. Couldn’t agree more Anya Chambers ! Yes we have been using our proprietary HipCloud personalized physiotherapy app to optimize non-operative, pre-operative and postoperative physiotherapy care plans in the Heuppraktijk / European Hip Clinic since more than 3 years now including >3500 patients. Our first (non published) results are truly amazing with 50% of patients with low to intermediate symptomatic grade 2-4 OA not requiring surgery for the first year. It is even up to >85% with FAI and DDH. Moreover, patients requiring surgery entered the OR with significantly enforced peri-articular pelvic muscles in comparison to the prehab measurements. SUFE is an acronym for Slipped Upper Femoral Epiphysis. It is a condition that can occur in the hip(s) in children around the teenage years but sometimes younger. In simple terms the upper part, or the 'head', of the thigh bone slides off the rest of the bone. It tends to occur more in overweight or very tall children, and children with low thyroid hormone or Vitamin D levels. When missed or neglected, these children have severe difficulty walking and hip deformities which will require major operations to reposition the hip better for walking. When picked early the treatment is a simpler operation to prevent the slip from worsening while the hormones and Vitamin D are checked and balanced. How can you suspect SUFE?: Any hip/ groin pain or limp in a young person should be investigated by clinical examination and plain Xray. This is best interpreted by a children's Orthopaedic Surgeon because early SUFE is easy to miss.
