Have you ever done something so impactful that it changed a patient's life? Have any of your patients ever been so grateful for something you did that they believe you "saved" their life? I am certain this is true for every clinician at least once in your career. Several patients have commented that "I walk on water" because I was able to help clarify a diagnosis/change a treatment plan/help patients better understand their disease or illness and empowered them to live their best life. But walk on water? We mere mortals can only appear to walk on water and in order to do that, you must know where the rocks are below the surface! In our quest to deliver affordable, accessible, and excellent primary care services at our Plum Health DPC clinic, we allow patients to self-schedule. ✅ In our modern world, patient self-scheduling **feels** like a huge time saver, a win-win for doctors and patients, and an overall no-brainer - the stats from our self-scheduling software gave the hard data/evidence of that time saving to back up those **feels** Last year, I had 760 patients take advantage of my self-scheduling system - over 250 working days, that's about 3.04 self-scheduled patients per day (I usually see 6 - 8 patients each day with the other patients being scheduled for follow ups from a previous visit). 📅 It looks like the software company is assuming that there would be about 7 emails exchanged to schedule one appointment and that it would take about 15 minutes to send/respond to these emails OR have a phone conversation to schedule. ☎️ So to close this out - I love allowing our Plum Health patients to self-schedule! ❤️ ❤️ ❤️ Does your primary care doctor, pediatrician, or family medicine doctor allow for self-scheduling? How much time would you save each year if they did? ⏰

It's crucial to consider medication-related adverse effects. Good job identifying the potential cause of esophageal irritation with Doxycycline and addressing it by advising proper administration with a full glass of water and an upright position. Communication with both the hospital staff and the patient played a key role in resolving the issue. #clinicalpharmacy #medicationsideeffects
Real-life case scenario 🥳🥳: Few weeks ago, a female patient was admitted to the hospital and I started her on oral Doxycycline. Later, they called from the hospital asking me if they should give the patient a second dose of IV Omeprazole as the patient is suffering from burning sensation in her throat that they think is related to GERD. I asked the nurse a single question: when did the symptoms start? And yes! The patient never complained of anything before Doxycycline initiation. When I talked to the patient, she told me that when she takes the capsule she suffers from these symptoms! (Listen to your patients, They’re telling you the diagnosis!) I informed the nurse that the next dose of doxycycline should be given with a full glass of water and have the patient sit up in an upright position for at least 30 minutes thereafter (1-2 hours according to the Canadian labeling). *Doxycycline might cause esophageal irritation, so it should be given as mentioned above to avoid such adverse event. Unfortunately, Many physicians and healthcare providers aren’t aware of such problem.

(How a Nurse’s Documentation Helped Overturn a Denial) When Mr. Jones was denied another round of home infusion IVIg for Myasthenia Gravis, I was asked to handle the appeal. I initially struggled to find strong progress notes to support our case. But then I looked at the nurse’s home infusion notes, which usually include data like heart rate and medication doses, pre-medications, and other quantitative information. To my surprise, this nurse had also recorded valuable observations about Mr. Jones’s life. She noted that Mr. Jones, who cared for his sick wife and had three grandchildren at home, was the only caregiver in the family. She also mentioned his love for gardening, which his condition limited but was greatly supported by the IVIg treatments. These personal details were crucial in our appeal, and thanks to this insightful documentation, Mr. Jones’s treatment was approved.
