Today, I’m grateful for grateful patients. Are you my advocate? That’s what a patient asked me. You explained that so well. It now makes sense. Thank you for advocating for me. She did not know whether her appeal would be upheld or overturned at that point. Working in health insurance as a nurse, that’s rare to hear. The majority of providers and patients think the nurse is only here to ask trick questions to get your case denied. The nurse reviews the medical record. The nurse may even ask for additional records for support. The facts are the facts. Some things are medically necessary and some are not. There are guidelines, standards of care, evidence based practice, etc that are utilized in the review. A medical record review is done thoroughly yet efficiently. Sometimes, there are extenuating circumstances that allows for approval for a medication, procedure, device or medical stay outside of the guideline. Sometimes the medical history is significant, the procedure is risky, or the medication is contraindicated. When this happens, you do really want someone to comb through and identify these risks even if it’s for a “simple” outpatient procedure. The provider can be alerted, just in case something was missed then referrals made for the appropriate evaluations for medical clearance. A fail-safe. Not all patients are the same. (Honorable mention Melissa Paige) We all know the medical system is beyond broken, not just for our patients but also for ourselves and coworkers. The exploitation of patients in a for-profit healthcare system and the labor rights of workers is intertwined. Obviously, in a for-profit healthcare system, the primary motive is to maximize profits. This often results in cost-cutting measures that can lead to under-staffing, inadequate facilities, and sub-standard care. This puts the burden on healthcare workers to meet the needs of patients… and the demands of a profit-driven system, often at the expense of their own well-being and job satisfaction. Labor exploitation of healthcare workers is proven to negatively impact patients. Workers who are overworked, underpaid, and lack job security are more likely to experience stress and burnout, leading to poor patient outcomes. On the flip side, healthcare workers who lack job protections often feel they cannot speak out against unsafe working conditions or report medical malpractice, which can harm patients. The exploitation of patients and workers in a for-profit healthcare system are interlinked, and addressing one issue can lead to improvements in the other. As a country, we need to move away from a for-profit healthcare model, as this will never be patient centered or encourage Just Culture. This can be done on a state basis until it is expanded federally, California is currently the most successful example. We need more bedside healthcare, workers, taking local office and leadership roles in hospitals. We need to start holding our professional organizations & politicians accountable, regardless of their political affiliation. If they take money from insurance companies / pharmaceutical companies / @ahahospitals they need to lose their job. Full stop period. Even if their opponent might be worse in the short term. We need to hold our representatives accountable, and show them they will lose their job if they do not work for us.

Are most physicians happy in medicine? Nope, not even close. In fact, most physicians are very miserable. This goes at odds with what most of the public think, that physicians are merrily working along because they get to do what they always wanted to do. Most medical students think that they will be happy once they arrive as an attending, only to find the opposite situation. Why? What you don't see is all of the other stuff that physicians are forced to do that we don't like. We have essentially been forced into high volume models of practice by health insurance companies and large hospital systems. We have become a commodity. The emphasis is on doing more and more with little care to the quality of the work being done. More time is spend on administrative garbage than actually treating patients. This is because all of the hoops that are required in order for health insurance companies to agree to pay. Physicians have become a commodity to a lot of large employers, who aim to squeeze every drop of productivity out of them, and then wonder why they burn out. At its core, medicine is a very rewarding and enjoyable field. Unfortunately for most, that core is covered by a thick layer of frustrating, time wasting, no clinical value add garbage. #healthcare #medicine #physician

Healthcare workers continue to leave medicine, not just in America but other countries. One view is: working people are failing the system… Another explanation and my opinion: the system fails the people, fails the dedicated workers, and therefore, they leave the system. Including me, there are literally hundreds of thousands of US healthcare workers, physicians, providers, nurses, allied health professionals, pharmacists and more…who are burned out, over stressed, under appreciated and leaving medicine for new careers or starting new careers in other related fields. The healthcare system wants to label you and place you into a box, a pigeon hole…then over work you, underappreciate you, emotionally drain you, and ask you to repeat again tomorrow. Then the system slowly erodes the very box that you have been placed into… Every year, your pigeon hole box has: -Less patient contact and direct contact time -Less bonus and perks -Less overall satisfaction -More administrative work due to unnecessary rules and regulations made by non-physician attorneys, medical boards, IPAs, hospitals, insurance companies and pharmacy managers. -Longer days and nights -Continued on-call staffing -Increased personal time consumption by administrative time interruption of patient care Increased patient noncompliance due to increased social media, conspiracy theories etc -Little focus on your mental health, physical, emotional, intellectual health, and ability to function as a provider and parent. This is the system structure that treats patients in American medicine. Hundreds of thousands of American healthcare workers have dedicated their lives to caring for people. They want patient contact and more contact. They don’t want more CME, more rules, more regulations, more barriers to care, more obstacles, less appreciation. They complain. They are not heard. Someday, they say. I’m done. I’m leaving at whatever cost. I’m one of them. Best decision I ever made. 🧐
