The impact of settler-colonialism, including the devastating legacy of residential schools, has left deep scars on Indigenous mental health. Today, many Indigenous communities continue to face significant barriers in accessing the care they need. Having previously developed and led a psychological staff support service in an NHS acute teaching hospitals Trust,this unfortunately doesn't surprise me. A good number of staff who were referred to me for assessment and therapy were often struggling due to systemic factors embedded in the NHS work culture. Managers wanted them to be more resilient and get back to work, but how do you achieve this when you are a human being and not a robot? It's not surprising then that moral injury comes into play when workplace culture and pressures drive a burnt out work force, who often receive little care and compassion themselves from management, into working like they are not humans. We need to value and take care of our NHS workforce, and create collaborative and respectful teams if we want them to be able to take care of us. Their human needs for regular breaks, annual leave, paid over time, investment in CPD, and overall working conditions need to be prioritised. The solutions lie in restructuring the dysfunctional systemic work culture on all levels, from primary care and community based teams to hospitals.

In reviewing potentially avoidable emergency department (ED) visits, I have seen many patients who have repeated ED use also have a mental health need that is not being adequately addressed. When building your preferred provider network in an ACO, are you including mental health providers? Is your care management team aware of the mental health risks and needs of the patient population? One thing I love about Acclivity is the Johns Hopkins ACG System that we use identifies patients with needs for mental health treatment as well as our event notification system (ENS) that shows patterns of ED use that indicate a potential unmet mental health need.
