In 2012, I led the development of a large telehealth program in a 100,000 square mile catchment area covering 2 western states. In these early days of telehealth, CMS would only cover rural originating sites and urban distant sites, and commercial insurance coverage was virtually (pun intended) non-existent. We had between 25 - 30 rural and frontier health communities and an equal number of urban specialty care sites enrolled in the program. We used to receive 10 - 15 patient testimonials per week, I would read and share every one of them with legislative, regulatory and policy making stakeholders so they would understand the positive impact that technology enabled care has on patients, families and caregivers. I'm very proud of the pioneering work we did, but I'm still frustrated that government and commercial insurance coverage is granted one CPT code, one modifier and/or one year at a time. It's 2024, not 1924, we have the technology and infrastructure to permanently eliminate barriers to all virtual care models. Now we just need leaders to lead, and create coverage and reimbursement parity between all technology-enabled and in-person care models.
