2moThis is a nice summary of where CMS continue to attempt to align acute and post-acute incentives, outcomes and measures, and provider engagement with each other and patients. CMS is building off the success of CCJR and the original VBP programs to continue to drive functional improvement. When people move better, they feel better, and when they feel better, they don’t need to access urgent or acute services as often. I don’t think it is a coincidence that TEAM will include postoperative conditions where post-acute care and improved engagement in care provides patients a chance to surpass their prior level of function.
