"Even when patients’ symptoms are made objectively worse by therapy, they tend to assume the therapy has helped. We rely largely on how “purged” we feel when we leave a therapist’s office to justify our sense that the therapy is working. We rarely track objective markers—for example, the state of our career or relationships—before reaching a conclusion. Sometimes when our lives do improve, it’s not because the therapy worked but because the motivation that led us to start therapy also led us to make other positive changes—such as spending more time with friends and family, reconnecting with people we haven’t heard from in a while, volunteering, eating better, and getting more exercise." The question still remains: if objective markers of therapeutic effectiveness show that therapy has helped a patient but the patient feels otherwise, or a patient feels therapy has helped when objective markers indicate otherwise, what should we make of this disagreement? I was reporting to a colleague just today the sometimes paradoxical experience in which I often get the most positive of feedbacks from cases where I know for a fact I wasn't doing much in terms of technical interventions. On the other hand, there've also been a few instances when I had gone overboard with some pretty good structured stuff but the feedback wasn't exactly commendatory. Who (or what) should decide if a therapeutic intervention is helpful and/or effective? Whose report should be prioritized: objective data or subjective report? The ideal scenario of course would be a situation where both objective measures of outcome agrees with client's evaluation (positively or negatively). When this happens, the therapeutic world is as it should be and everyone is on the same page. Though for ethical and empirical reasons, I lean towards privileging and prioritizing objective markers of therapeutic effectiveness. But my clinical experience is a bit more messy in terms of what is subjectively valued and what objective criteria of efficacy stipulates. https://lnkd.in/dUANpdgq
