Big news for pancreatic cancer therapy this week! The FDA approved irinotecan liposome (Onivyde) in combination with oxaliplatin, leucovorin, and fluorouracil (NALIRIFOX) for the first-line treatment of metastatic pancreatic adenocarcinoma. Approval was based on results from the phase III NAPOLI 3 trial involving 770 patients. According to this update in the Oncology Nurse Advisor, the FDA recently approved a drug combo of Krazati® (adagrasib) with Erbitux (cetuximab) for the treatment of adult patients with KRAS G12C-mutated locally advanced or metastatic colorectal cancer (CRC). Of the 94 patients enrolled in he study, 34% had an overall response rate (ORR). According to the National Library of Medicine, "The tumor objective response rate (ORR) is the assessment of the tumor burden (TB) after a given treatment in patients with solid tumors and has a long history. The ORR is undoubtedly an important parameter to demonstrate the efficacy of a treatment and it serves as a primary or secondary end-point in clinical trials." 🚀 Exciting News: FDA Grants Full Approval to Tepotinib for NSCLC 🎉 Big news in the fight against non-small cell lung cancer (NSCLC)! The FDA has granted full approval to tepotinib (Tepmetko) for treating NSCLC with mesenchymal-epithelial transition (MET) exon 14 skipping alterations. This follows its accelerated approval in February 2021, backed by strong data from the VISION trial (NCT02864992). In the VISION trial, tepotinib showed promising results, with an overall response rate (ORR) of 57% among treatment-naive patients and 45% among previously treated patients. Many responders experienced a durable response of 12 months or longer, offering hope to patients, particularly older individuals often underrepresented in trials. Dr. Paul K. Paik from Memorial Sloan Kettering Cancer Center highlights the significance of these findings, especially for older patients with limited treatment options. The median overall survival of 20 months is encouraging and emphasizes the importance of advancing precision medicine. Tepotinib's approval marks a significant step forward in NSCLC treatment, emphasizing the value of targeted therapies in improving patient outcomes. Today I wanted to share this article that I read from the Cochrane Library, published 2 months ago. Platinum‐based chemotherapy for early triple‐negative breast cancer Summarized Information: Background: - Triple negative breast cancer (TNBC) makes up ~ 15% of all breast cancers. It affects African American and Hispanic woman at higher rates with worse outcomes. 5 year survival rates are as follows: 91% (localized), 66% (regional), 12% (distant), and 77% (all stages combined). [Reference: Breast Cancer Research Foundation and the American Cancer Society] Objective: - Systemic Review to evaluate platinum‐based adjuvant and neoadjuvant chemotherapy (carboplatin) benefit and risks in patients with early TNBC. Methods: - Standard Cochrane methods, only randomized controlled trials, included 20 published studies involving 21 treatment comparisons and 25 ongoing studies. Primary Outcome: - Disease‐free survival (DFS) and overall survival (OS). Results: - Survival outcome follow-up ranged from 36-97.6 months. - Disease Free Survival --> hazard ratio 0.63, 95% confidence interval 0.53 to 0.75 - Overal Survival --> hazard ratio 0.69, 95% confidence interval 0.55 to 0.86. Conclusion: - Platinum‐based adjuvant and neoadjuvant chemotherapy (carboplatin) has been shown to improve both DFS and OS. However, there were increased chemotherapy delays and dose reductions.
