February 28 is HCM Awareness Day! Hypertrophic Cardiomyopathy (HCM) is the most common inherited cardiac and affects an estimated 1 in 500 people. HCM causes the heart muscle to become abnormally thickened and stiff. This can make it harder for the heart to relax and fill with blood, as well as limit its ability to pump blood out to the rest of the body. People with HCM can experience debilitating symptoms including breathlessness particularly during exertion, chest pain, tiredness, fainting and palpitations. Here is a new case by Neil Sardesai, MD FAAOS and Kaiser Permanente. ISOLATED PROXIMAL ULNA FRACTURE IN 43M A 43-year-old right-hand dominant male has a low-energy fall from a sitting position onto his left elbow. He presents to our emergency department with difficulty with elbow range of motion and focal pain over the left proximal ulna and ulnar shaft. He Denies any numbness or tingling. At baseline, he requires an assistive device for ambulation (either a walker or cane). Of note, the patient has severe hypocalcemia (6.1 mg/dL [8.5 - 10.7 mg/dL]) at the time of ED presentation and requires correction before proceeding to the operating room. PMH The patient has significant medical co-morbidities including uncontrolled Type 2 diabetes and end-stage renal disease requiring maintenance dialysis three days a week. He also has renal osteodystrophy, hypertension, and severe hypocalcemia. PE On physical exam, he is in no apparent distress. He is pleasant and communicative. His AIN, PIN, and ulnar nerve motor exam are intact in his left upper extremity. There are no open wounds. He has tenderness over the left proximal ulnar diaphysis. 📝 𝐂𝐚𝐬𝐞 𝐑𝐞𝐩𝐨𝐫𝐭: Occipitocervical Fusion for Atlantoaxial Instability Secondary to Post Laryngeal Procedure Infection in a Down Syndrome Patient 🖋𝐀𝐮𝐭𝐡𝐨𝐫𝐬: Romulo Augusto Andrade de Almeida, Fellipe de Paula and Andrei Fernandes Joaquim A 7-year-old girl, previously diagnosed with Down Syndrome (DS) with associated moderate cognitive impairment, was brought to the emergency room with a 30-day history of opioid-refractory neck pain with painful restriction to head rotation and fixed head position. Additionally, she presented hypoactivity, eating avoidance, and fluctuant fever. At initial neurological evaluation, there was tetra paresis (grade III in both superior and inferior limbs) and mild spasticity.
