Dear CDC, Re: Instrument #Sterilization Lapses in Florida? The #CDC's report in #MMWR discussing this cluster (link: https://lnkd.in/eRkRUrEv) is as notable for educating the public about the risks of exposing patients to nontuberculous #mycobacteria (or #NTM) during cosmetic surgery performed in the U.S. as it is (in my opinion) for the crucial data the health officials did not disclose and/or did not collect. The CDC's report, for instance, does not identify the #Florida clinic by its name. This omission raises a number of questions -- including the potential for publication bias. Generally speaking, disclosure and transparency are integral to public health's mission. (Also oddly, a "state health department spokesperson said the agency doesn’t confirm or deny the existence of doctor licensure or epidemiological investigations," according to an associated new report in the 'Tampa Bay Times,' which is appended.) Second, according to the CDC's report, the source (of the cluster's M. abscessus) "has not yet been identified." Presumably, the tap water was a suspect. Yet, the report does not state specifically whether investigators microbiologically cultured the clinic's tap water, and if so, what the findings were. (It is unclear whether such environmental testing might have been precluded during the investigation once the clinic was closed.) And third, the CDC's report states that health officials identified "gaps" in #infectioncontrol -- including "surgical device #disinfection." But what exactly were these identified lapses that could pose an increased risk of NTM infection during cosmetic surgeries? We need each misstep identified so #infectionprevention practices in other U.S. facilities can be promptly audited, and corrected as required, to ensure the same breaches are not being repeated. Listing the identified gaps seems basic to me. While conjecture, I wonder whether the clinic might have been disinfecting, rather than sterilizing, one or more reusable surgical instruments followed by rinsing them with NTM-contaminated tap water, and then using the instruments (if inadvertently) wet without first drying them. Cross-contamination, the transfer of harmful micro-organisms during or after surgery, often leads to surgical site infections (SSIs), which can be caused by bacteria, fungi, parasites or viruses and spread through various means, including unsterilised medical equipment, human contact and contaminated objects. Such infections can complicate and delay healing, increase pain, prolong hospital stays, escalate costs and increase chances of morbidity and mortality, especially in immuno-compromised and older individuals. Cross-contamination can occur during procedures such as surgery or IV administration, making proper hygiene, wound care and sterilisation methods crucial for prevention.
