The IU West Hospital Cath Lab/Interventional Radiology department provides patients with access to lifesaving treatments, cutting-edge technology, and research-based medicine. The team cares for adult patients using teamwork, strong critical thinking, and quick interventions. The RN provides hands-on care for patients undergoing diagnostic heart caths, interventional cardiac caths, and electro-physiology procedures as well as patients undergoing Interventional Radiology procedures. In the cath lab, nurses have the opportunity to use their nursing skills and experience to make an immediate impact on our patient's lives.

Performs inpatient/outpatient treatment procedures and physician orders. Operates applicable hospital equipment. Continuing our exploration into the nuances of nasogastric (NG) tube management, today's focus shifts to the innovative devices designed to assist with NG tube insertion. The right tools can significantly enhance the safety and efficiency of this common yet critical procedure. 🛠️ Assistive Devices for NG Tube Insertion and Possible Common Complications Related to Their Use The advancement of medical technology has provided us with a range of devices that can support the accurate placement of NG tubes. These assistive devices are engineered to reduce patient discomfort and minimize the risk of complications associated with blind insertion techniques. 💭 Guide Wire-Assisted Placement Devices: 📍 These devices utilize a soft guide wire to help navigate the NG tube through the upper GI tract. The guide wire can reduce the risk of misplacement into the lungs, which is crucial for patient safety. 📍 There is still some risk of improper placement. The guidewire makes the tube heavier and more challenging to kink, but placement in the lung is still possible. 💭 Electromagnetic Tracking Systems: 📍 Sophisticated tracking systems offer real-time guidance and visualization of the tube's trajectory. This technology aids clinicians in avoiding anatomical obstructions and in confirming that the NG tube reaches the correct location within the gastrointestinal tract. CORTRAK is the name I am most familiar with. 📍 CORTRAK was recalled in March 2022 due to incidences of patient injuries and death. The recall focused on the improper placement using this system, which did not require chest X-ray verification unless the device told the inserting provider they needed it. Due to the recall, every CORTRAK-inserted NGT must be verified with a X-ray. 💭 pH Testing Equipment: 📍 Once the placement is assumed to be correct, pH testing equipment is vital in verifying that the NG tube's end is indeed within the stomach. This non-radiologic method provides a quick placement confirmation. 📍Unfortunately, pH verification has faults; there may be false pH readings in patients taking certain medications or having certain medical conditions. 💥 THE GOLD STANDARD FOR NASOGASTRIC TUBE PLACEMENT VERIFICATION IS A CHEST X-RAY. 💥 👩⚕️ Nursing Integration: Nursing staff needs to be proficient in using these devices. Training and ongoing education ensure that nurses can effectively incorporate these tools into their practice, thus enhancing patient care. 📋 Protocol Development: Healthcare facilities must develop clear protocols for using these devices, including when and how they should be employed, maintenance of the equipment, and troubleshooting any issues that may arise during insertion. A U.S. Department of Veterans Affairs Medical Center in Colorado paused heart surgeries for 13 months, and its leaders failed to inform higher-ups. The Department of Veterans Affairs, Office of Inspector General found that the VA medical center in Aurora, Colorado, paused cardiac surgeries for over a year in 2022-2023. The leadership cultivated a "culture of fear" at the hospital, leading to the departure of critical nursing staff. The pauses in heart surgeries were not directly linked to patient harm, but the toxic work environment jeopardized patient safety. The facility was forced to pause heart surgeries again two months later after continued efforts to attract permanent personnel failed. During the second pause, the hospital's entire cardiac surgical staff left. The hospital was able to resume procedures only 13 months later after it contracted with the University of Colorado to provide surgical teams. This incident highlights the importance of effective leadership, staff retention, and transparent communication within healthcare organizations. It is with great delight we share that Austin Health ICU Research Nurse, Dr Glenn Eastwood, has been published in the July edition of the New England Journal of Medicine (NEJM), the world’s most prestigious medical journal. Dr Eastwood is the Chief Principal Investigator on the global TAME trial, and is first author on the NEJM publication titled ‘Mild Hypercapnia or Normocapnia after Out-of-Hospital Cardiac Arrest’. Publication in the NEJM is a milestone achievement that reflects the long-term dedication of many people, in many countries, over many years. “To be able to successfully lead this international trial is a career highlight and one that I hope will inspire others to be active clinician-researchers,” says Glenn. With brain injury being the leading cause of death and disability in patients admitted to ICU after cardiac arrest, interventions to reduce the severity of brain injury after cardiac arrest are desperately warranted. The TAME trial addressed the question of whether targeting about 20 per cent higher than normal carbon dioxide (CO2) levels in the blood (mild hypercapnia) - aimed at increasing blood flow to the brain - compared to targeting normal CO2 levels (current guideline-supported practice) could improve neurological outcomes in patients admitted to the Intensive Care Unit (ICU) after Out-of-Hospital Cardiac Arrest (OHCA). While it may seem counterintuitive, it was thought that maintaining slightly elevated levels of CO2 in the brain may promote improved blood flow to the brain, resulting in better oxygen delivery that would ultimately lead to better neurological outcomes. After recruiting 1,700 patients from 63 intensive care units in 17 countries, results showed that targeted mild hypercapnia did not improve neurologic outcomes at six months as compared to targeted normocapnia. Importantly, the incidence of adverse events was similar in the two groups and were aligned with other related trials. "It was a really interesting hypothesis to test, as based on current physiological understanding, the theory held some merit, and there had been preliminary evidence to support it," says Glenn. He adds: "While targeting mild hypercapnia didn’t lead to improved outcomes, our study provides the strongest evidence to date, and indicates no benefit to changing current practice."
