I never used to be on board with direct primary care. That was before the massive mess we see from a combination of private equity running healthcare, the Change Healthcare disaster freezing payments and causing practices to go bankrupt, hospital networks shutting down maternal and child health units for higher margin services, insurance raising rates (charged in premiums, deductibles, and co-pays) while lowering reimbursement, etc. Doctors employed by bankers and investors are, almost always, forced to violate ethics and end up with burnout and moral injury. Also, private equity extracts value, lowers safety and quality, then shuts down hospitals and clinics, sells off real estate to liquidate assets. reduces access, and overall “disrupts” in ways that harm.

Primary care is in complete disarray. While we need policies to promote hybrid payment models, we also need innovation. We need a completely new and separate “science of primary care”. A science that looks at patients as complex biological systems living in complex dynamic environments. We also need a new understanding of the term health “system”. We need seamless care delivery that mirrors the natural cycles of wellness and disease states that occur as patients navigate changing environmental conditions. Those health systems should be based on a new and expansive primary care model focused on the prevention and early detection/mitigation of disease processes. Our current medical system with its associated payment structures has it completely backwards. We need to think differently.
