We need to STOP talking about treating patients because in your next role you’re not going to be doing that And instead we need to START talking about more business and operational initiatives you are owning Does this help?

The lack of medical specialists in rural America is a big problem. One of the … primary causes of this shortage is something I like to call “White Knight Syndrome” (WKS). It’s a rampant disease and is hurting patients. Let me explain. WKS is caused when well intended healthcare systems falsely hope that a specialist is going to ride into their community, set up a practice for 30 years, and never leave. Sounds wonderful, but it only happens in story books. But wait Dr. P, why can’t I recruit a full-time specialist? Here’s why: 1. No one is going to agree to “only 10 days of ER call a month”. Change your outdated bylaws please and improve your job posting. 2. Most of us value quality of life over quantity of our paycheck. Your community is lovely, but we’d rather live in Nashville, Atlanta, Charlotte etc. No amount of money will change this fact. 3. Healthcare is a team sport. It’s really challenging to recruit additional staff to join you in rural communities. We can’t do this alone. What’s a solution you may ask? Innovative care models. Partner with companies like VirtuCare who implement outpatient clinics by leveraging telemedicine and periodic on site care. The other week I posted about companies claiming to be “#militaryspouse friendly” but only offering entry-level positions despite the applicant being overqualified. And last night someone commented the below. It is this archaic, uninformed mindset that continues to plague my community and it needs to 🛑stop🛑. So I want all the #educated and #skilled #militaryspouses to use the post to demonstrate how wrong this commenter is! Let me start: ✴️ I am a Doctor of Physical Therapy with 10 years of experience in ICU, Level 1 Trauma, and Burn rehabilitation (as well as stroke, cardiac, med-surg, orthopedic, etc). ✴️ While a clinician, I led multiple electronic health record improvement projects to include a system-wide documentation environment reorganization and optimization. ✴️ I have worked full-time in health IT for over a year now and facilitated the transition of the Lovell VA from CPRS to Oracle Health Cerner Millenium. ✴️ Additional current studies include data analytics and coding. ✴️ I have experience in adult education as a clinical instructor for 8 DPT students and trainees. ✴️ Outside of work I serve as the communications chair for the Academy of Acute Care Physical Therapy, a section of the American Physical Therapy Association (APTA) , enhancing the quality and content of information disseminated to members. I am also on committees optimizing acute care practice and developing tools for acute care clinicians. The list could go on and on. I know I am skilled. I know my network of military spouses are skilled. The are your confidant They listen without judgment They hold space for you to rant and rave or ask questions to figure out a situation That one person that you can call Say hello to your clinical supervisor! A vital person to have in your work life as a therapist. Because not only does it stop you bottling up everything, it stops you feeling isolated And most importantly they understand and know exactly what you're going through. As your VA and for confidentiality reasons, you can't talk to me about all the deep clinical stuff you work through with your clients. And to be honest, I reallllyyy don't wanna know. But your clinical supervisor is there for you as your sounding board, to discuss cases and if you need to tweak anything in your modality. What does the term Patient Engagement mean to you? To your work as a medical professional or patient advocate or to your life as a person we want to engage? I’m asking seriously because on LinkedIn, where #patientengagement is a very popular hashtag, it seems there is little to no agreement on what it means. For some, it’s a method of increasing #customers; for others, a way to raise #patientsatisfaction scores; still others are hoping to teach patients how to advocate for themselves; or empower them; or make #shareddecisionmaking the gold standard. So, please, share your voice and comment below how YOU define the term. Maybe, we just need alternate terms... Let’s see if we can come to a consensus. Keep it simple please. Another form of gaslighting happens to medical practitioners themselves: When the physician is the patient, between physicians, between physicians and other members of the health care system, and between physicians and themselves. Such medical professional gaslighting has received little attention to date. Gaslighting occurs when there is a gaslighter- who must be right at all costs, and a gaslightee- who needs the approval of the gaslighter at all costs. It can occur on a macro level with the entire healthcare system, or a micro level between two individuals. Medical professionals are gaslit (often unintentionally) throughout their careers, a form of survival in an unsafe environment, and then unintentionally transfer learned behavior to patients, directly and indirectly, with time-honored beliefs:⁣ ⁣ Never show weakness.⁣ Never make a mistake, and smother it if you do.⁣ Respect the hierarchy.⁣ ⁣ Deviation from these tenets puts practitioners in the crosshairs of scrutiny. ⁣ Gaslighting a patient or medical professional is easy to do: simply plant a seed of doubt by making them feel wrong, weak, or crazy. The result is deep shame (“not enough-ness”), which manifests as constant survival mode behavior: ⁣ ⁣Am I OK? ⁣ What’s right? ⁣Who’s right? ⁣ Am I doing enough? ⁣Am I doing it wrong? ⁣ Is someone judging me? ⁣ Is my doctor wrong? ⁣ Will the patient find out? ⁣ Will I lose my job? ⁣ Am I safe? ⁣ ⁣ Gaslighting is especially likely to happen to women and people of color.⁣ ⁣ Medical centers are highly emotional environments; they are the sum of people’s experiences within them. Amidst other systemic issues in American health care, the permeation of shame in medical culture, in combination with a lack of formal training around how to cope with it, has far-reaching deleterious implications for practitioners and patient care.
