Camden Coalition has a bit of a cult-like status within complex care models, and the work there is ground-breaking, mission-oriented and thoughtful. Given my admiration for them, the studies showing their lack of demonstrable results point to a simple fact: Care coordination alone, even when implemented well, is insufficient to reduce hospitalizations for this [really, any] complex population of high-need patients. Primary care has proven its value. I am more concerned that a lot of the innovative, Specialty-focused models seem to rely on navigation, coordination and peer supports - without learning the lessons that are tried and true about complex populations. The reality is that these populations actually need a place to go when their needs escalate, a primary care provider to help them and real specialist access/integration for their needs. Let's hope that the next wave of these innovative companies learn these old lessons - these populations deserve it. https://lnkd.in/ecsjcbY2

Exciting news for all people with T1D in the UK. NHS to offer people with type 1 diabetes an artificial pancreas (Hybrid Closed Loop System). It is just the beginning, hope to see soon this will be rolled out in other parts of the world. A world-first trial on the NHS showed the artificial pancreas was more effective at managing diabetes than current devices and required far less input from patients. Final draft guidance from the National Institute for Health and Care Excellence (Nice) recommends that people in England and Wales should benefit from the wearable device if their diabetes is not adequately controlled by their current pump or glucose monitor. “Using hybrid closed-loop systems will be a gamechanger for people with type 1 diabetes. The health and care system in England must shift its focus away from hospital care to primary and community services if it is to be effective and sustainable. Despite successive governments repeating a vision of health and care services focused on communities rather than hospitals, that vision is very far from being achieved. This research found that to achieve the vision, political and other national leaders will need to completely shift their focus away from hospitals towards primary and community health and care – and all policies and strategies must align to that focus. #nhsengland #nhs #healthcare #primarycare #acutecare #communitycare

New report this week from The Kings Fund highlighting all the challenges to shifting care left, out of hospital and towards primary, community and prevention. So many things have to happen to enable this, it seems overwhelming. However, right at the heart of the report data gets a mention. Whilst the data we work with is never perfect, linking it all up for systems, adding in data on the wider determinants of health and presenting it in the right way is the start of changing the conversation. It's the start of enabling teams to work differently. It's the beginning of thinking differently and planning for about future population and workforce needs. It's a new perspective and a move to view population, people and needs rather than organisations, diseases and activity. It's a way to look back and recognise the progress teams are making towards outcomes shared across system providers and local communities. Whilst the data is never perfect, without joining it up, it's hard to get started. I work in a team who are passionate about linking data, keeping it simple and using it to drive changes like those described in this report.
