Providing more wide-ranging support to dyads led to a much better care experience. 🌟 Support with care navigation is a critical, unmet need currently being taken on by caregivers and contributing to caregiver burden. 🤗 The Importance of Quality of Life Metrics in Healthcare ❤️ One of my professional goals is to provide high quality QoL compliant data in terms of physical, emotional, value-system, historic and psychosocial measures through decision trees to improve healthcare interventions. 📋 Why #QoL Metrics Matter 🍀 1️⃣ Improved Clinical Care: Hip replacement patients experience a 10% higher rate functional improvement and recovery compared to those without QoL monitoring. Patients with #depression with QoL-aligned interventions showed a 25% reduction in depressive symptoms within six months. And in cardiac rehabilitation, we see a 30% improvement in physical functioning! (Journal of Arthroplasty, Journal of Affective Disorders, American Journal of Cardiology) 2️⃣ Healthier Care Ecosystem: In terminal cancer patients, #palliative care programs let by QoL led to a 25% improvement in patient comfort and a 20% reduction in caregiver stress. QoL-informed care plans ripple out in this way, improving not just the lives of those with the disease but the lives around them. (Journal of Palliative Care) 3️⃣ Demonstrated Societal Impact: Children with chronic illness whose care plans were adjusted based on QoL, demonstrated a 15% improvement in school attendance and a 20% enhancement in physical and social functioning. (Journal of Pediatric Healthcare) 4️⃣ Enhanced Clinical Metrics: From a pharmacological approach, if a new medication is added and suddenly the patient is experiencing insomnia or new behaviors, that is vital data to capture longitudinally. QoL metrics can also capture comprehensive insights into a patient's overall health, such as 'how many steps can the person take?' That question may seem small, but it measures the difference between needing a home aid or not to help get to a bathroom. 5️⃣ Reduced #Healthcare Costs: QoL-led care in post-stroke patients resulted in a 20% higher rate of return to independent living, as well as a 15% reduction in falls over one year. QoL Care Plan implementation reduces the burden of care on the patient, caregiver and system itself. (Stroke Journal) 6️⃣ Happier Patients: Close to 30% of people drop out of treatment all the time for reasons that are personal not clinical. However, breast cancer patients who received tailored support based on QoL assessments reported a 20% higher overall satisfaction with their treatment. It is no surprise that in #strokerehabilitation "The number of PT providers correlated negatively with QI Mobility discharge scores (r = −0.41, P ≤ 0.001)?" For our patients and our families, we are typically the first to encourage and support a return to "normal" and the freedom that mobility brings. We as #neurorehabilitation #physicaltherapists and #occupationaltherapists work hard to establish a well-placed sense of trust that we are not going to allow our clients to fall when working with us. This trust takes time to build and requires getting to know our client's strengths and weaknesses. This is not a one-day process. Imagine, if you were completely dependent on someone else for your ability to move. Would you find that you are fearful of falling and injuring yourself? Now imagine how many times with each clinical transition you would experience that. Would you find this overwhelming to start over and over with a new person? One of my favorite roles as a clinician was one in which I was able to sharpen my clinical skills the most as a "float" therapist and KEEP my clients! I was happy to be "rigidly flexible". My amazing supervisor put the client first and had me follow and treat the same patient from the ICU, to Acute, to #irf, and then #outpatient. She was ahead of her time. After initially building rapport with a client and their loved ones, there was not a moment wasted with each transition. Instead, we could pick up exactly where we left off and make significantly more progress than if my clients had 4 different therapists. Additionally, I became a much better therapist because I could see how all of the training during the hospital stay impacted the client after she was finally able to go home and reported her observations back to me in outpatient for a continuous process improvement of both the therapy team and client experience. Why do these transitions occur? Doesn't a client have enough to deal with? Are we able to imagine a model like my innovative supervisor that serves our communities and not the policymakers that can permit a care team to follow a client from admission to home.? How much more recovery would be possible? What would our clients #outcomes across all domains not just therapy look like? Less medical errors, less client caregiver frustration, and increased adherence? Increased staff retention perhaps with increased satisfaction in being able to focus on our clients. What does your ideal scenario look like to be able to work with your client through their transitions through the continuum of care to improve their outcomes? How do we implement the changes needed based on the research? I would love to see and share your thoughts on this.

40% of states are considered “dementia neurology” deserts where access to care is minimal, especially in rural areas. Even in populated areas, receiving expert care can be quite difficult if patients have issues with transportation access or if they have difficulty leaving home.  Telehealth options can bridge this growing need for these patients.  Isaac Health is a digital health and in-person clinical service platform for patients with dementia and other brain health conditions. In addition to screening, diagnosis, treatment, and ongoing care, patients, their support teams, and clinicians can manage medications, cognitive rehabilitation, counseling, and psychosocial support.  Isaac Health partners with health systems and plans to offer its service and is fully covered by insurance.  As treatments are being developed for Alzheimer’s, early detection and proactive care become more important as it can delay cognitive decline by up to 3 years.  Isaac is live in about 15 states with plans to add six more states in 2024. We're proud to announce the deployment of a new cognitive screening tool that is outperforming traditional screening methods! Cleveland Clinic has introduced the Cleveland Clinic Cognitive Battery (C3B), an innovative tool, using technology developed by Cleveland Clinic and licensed to Cleveland Clinic Innovations portfolio company, Qr8 Health. C3B is a self-guided cognitive screening tool, which includes the Visual Memory Test and Processing Speed Test, for Medicare patients aged 65 and older during their Medicare Annual Wellness Visit which can aid in early detection for dementia.    “In the realm of healthcare, the need for early detection of dementia remains a crucial yet unmet necessity. I am beyond excited to announce that my research with Marissa Bergh, BSN, Tina Sadarangani, and Komal Patel Murali, PhD, RN, ACNP-BC has been published on the Journal of Medical Internet Research (JMIR). I want to thank Dr. Sadarangani for the wonderful opportunity to participate in this project. The skills and information that I have learned throughout working with this team has enhanced my interest in geriatric nursing research. Our article focuses on the use of a mobile healthcare application, CareMOBI, which aims to improve communication between dementia caregivers, providers, and healthcare professionals in the Adult Day Services (ADS) setting. The study evaluated the feasibility and acceptability of CareMOBI in four different domains: ease of use, clinical value, fit within workflow, and likelihood of adoption. https://lnkd.in/g-eWD-GN

June is Alzheimer’s & Brain Awareness Month!     As the population ages and healthcare costs continue to rise, there is a critical need to develop sustainable care models to improve population health while managing the total cost of care. Industry leaders from the Brain Health Leaders Network (BHLN) and Population Health Leaders Network (PHLN) consistently pilot and evaluate best practices that can be scaled to other organizations.   Recently, BHLN member John Sawyer, PhD, ABPP-CN co-authored a piece on dementia care in AJMC - The American Journal of Managed Care. Dr. Sawyer's research found that a 12-month dementia care management program resulted in a mean savings of $475.80 per member for a managed Medicare population.    Read the full article here > https://lnkd.in/eMRb5v5d   BHLN and PHLN are part of TKG's Networks of Excellence. 🚨 The need for virtual physician systems of care is becoming increasingly apparent due to physician shortages and rising rates of strokes. As we interact with hospitals and health systems, it's clear that many are already planning for or experiencing these shortages. 🔍 We're commonly seeing the following scenarios as organizations wrestle with these challenges: -Loss of coverage with little notice (sometimes with 30 days or less of notice) -Impending physician retirement with limited prospects for replacement -Robust coverage in some areas, but shortages in others (e.g., robust ambulatory capacity, but shortages on the inpatient side) -Full coverage, but physicians working at an unsustainable pace It's crucial for healthcare organizations to adapt and implement innovative solutions to ensure continuous, high-quality patient care. Integrated psychiatric care We have long advocated and supported the integration of psychiatric care into general physical health care. Here at Carilion, our department’s staff and faculty are integrated into treatment programs with (for example) family and community medicine, emergency medicine, internal medicine, geriatrics, pediatrics, obstetrics and gynecology, neurology, pain management, and acute hospital-based sepsis care. The field is now broadly recognizing the value and importance of such programs in terms of improved patient outcomes, reduced total cost of care, increased access to behavioral health services, improved patient and clinician satisfaction, and reduced stigma and disparity.
