Recent case demonstration of XO Score using low-pressure inflation to crack and dilate calcific stenosis in the left iliac artery. XO Score was used with an 8mm x 40mm off-the-shelf PTA balloon. No vessel spasm, dissection, or recoil was evident post-treatment and the entire procedure was finished in 33 minutes. XO Score is indicated to treat both fibrous and calcific lesions in the peripheral vasculature as well as AV dialysis fistulae. It works with a broad range of PTA balloons, offering controlled, concentric dilatation at 1:1 vessel sizing. Why are some of you guys are still doing this nonsense left ventricular angiograms with a single hole 🕳️ catheters and playing woth the fire 🔥 and risking your patient’s lifes? It can cause ventricular rupture, Thebesian veins injury even stroke? Please 🙏 stop 🛑 doing this. Thank you 🙏. First of all nowadays tgere is only few true indications for left ventricular angiogram as we can get most of the information via a good echocardiogram but also if you still want to do left ventricular angiogram, please use multihole pig 🐖 tail catheter and a medrad left ventricular angiogram injector with 600 psi, 1.2 rise or so with your desired contrast volume. Try to avoid left ventricular angiogram in patients with already elevated LVEDP. 🫀

68 years old male , Diabetic, HTN , IHD , Ex_smoker, and has history of Rt Above knee amputation. He presented with Lt CLI and patches of dry gangrene at toes. Patient has history of previous 3 endovascular intervention at another hospital CTA show patent iliac stent and occluded SFA stent . On Examination, only Lt femoral Puls. Transbrachial approach , crossing occluded SFA stent using GW 0.018 + supporting catheter Bail Out of SFA stent using rotational athrectomy device ( Jetstream) followed by DCB the result was suboptimal. I did prolonged balloon dilation at distal end and 2 extra pass of the device which give us potential satisfying result #athrectomy #occluded_stent #vascular

Echogenicity of peripheral venous catheters... "The echogenicity of a device is an essential component of successful ultrasound-guided peripheral venous catheter placement. Optimizing catheter intrinsic factors such as components of the cannula and length, and extrinsic factor like the insertion angle should be considered in their design and use to reduce puncture failure rates" Slosse et al (2024).

🟡Case today: ▫41 y.o female patient , post MVR and TV repair (De-Vega) 1 year ago, came complaining of dyspnea NYHA class IV, abdominal distension, yellowish discoloration of eyes and skin and palpitation. O/E: pt looks ill, jaundiced, pallor, bilateral pitting LLO, JVP raised. BP: 85/60 mmHg, PR: 132 b/m On auscultaion: S1 (metallic sound) + S2 + S3 + pansystolic murmer on tricuspid area Chest: Bibasal crepitations Echo: No signs of prosthetic valve failure, aneurysmal LA, spontanous echo contrast, lack of coaptation of tricuspid valve with moderate to severe tricuspid regurge (sign of TV repair failure) , and no pulmonary HTN. #annonucenter #annonu_center #cardiac #ecg #echocardiography

*PRP Injection for Elbow Ulnar Collateral Ligament Tear? Yes please! Patient has been symptomatic for over 6 months....failed one prior steroid shot and failed bracing. We used ultrasound to diagnosis a significant UCL tear and then to help do a guided PRP injection into the origin/insertion of the UCL and some even into the joint. We drew 54 cc of blood / 6 cc of ACD-A. Starting platelet count was low normal at 145....PRP platelet count was 1504 in 3 cc of PRP....pretty good. Very slightly LR-PRP with a WBC of 7.9 which is still within normal human range. Minimal granulocytes or RBCs. Fingers crossed this helps! The best published data suggests that most will get better with a single injection done with this technique.

In this case, the popliteal artery was recanalized by use of a 2.1. Jetstream rotational catheter. Prior to intervention, a small filter was placed in the tibioperoneal trunk to stop emboli. A blocked filter was present already after atherectomy but the intervention was finalized by additional angioplasty over the filter wire. Then, a 6 F guiding catheter with a Tuohy-Borst adapter attached was advanced to the filled filter system. Aspiration was performed via the adapter and the filter was pulled into the guiding catheter and both were retrieved together. No peripheral emboli were detectable afterwards.. This can be done with different types of filters. For aspiration, guiding catheters as well as aspiration catheters may be used preferentially with the largest possible diameter.
