Emergency physicians report the highest burnout rate (65%) among all specialties, according to a Medscape survey. In a MedPage Today article, Jesse Pines, MD, MBA, MSCE, and Amer Aldeen, MD outline the factors harming emergency medicine working conditions: -Sicker and more complex patients, post-pandemic -Nursing shortages -Increased hospital boarding -Diminishing reimbursement However, Pines & Aldeen see potential to turn things around: “Some say burnout is destroying emergency medicine. We beg to differ -- we believe there are tangible ways for hospital administrators, government, and physicians themselves to combat emergency medicine burnout and foster success.” Key elements of the turnaround: -Address ED Inefficiencies -Promote Career Development -Advocate for the Specialty -Increase Resiliency The article concludes, “We are living in a dark time in emergency medicine's history. Listening to negative voices will amplify, not reduce, this darkness. The road to success for emergency physicians is paved not with victimhood, but with empowerment. I get asked often, “How come so many emergency doctors end up going into leadership positions?” Since it’s Hospital Week, that’s a great question! The thing about emergency medicine is you get to know everyone. You’re calling doctors of practically every specialty. And you get to know many of the critical services in the hospital, like the intensive care unit or the operating room (after all, you sometimes transport patients there), and ancillary services like the lab and radiology. You’re always walking over to the CT scanners or the hematology section (because you want to know if they’ve looked at the lumbar puncture yet), or chatting with the pharmacists when they are restocking the med cabinets or hanging medications you ordered. When you get to know everyone, it’s the start of trust, relationships, and good will – all of which are invaluable forms of professional capital. And emergency medicine people also like to solve problems. You might want to get laboratory tests back faster, or figure out a better way to deal with the inevitable bureaucracy of hospitalizing someone. You might want to get antibiotics faster, or standardize how we sedate patients, or arrange for alcohol detox, or improve psychiatric transfers. Sometimes it seems like every problem under the medical sun lands in the ER and part of your job becomes helping to solve it - systematically. That means you get to know nursing leaders, department leaders, medical staff leaders, and eventually, hospital leaders. A final big reason is service ethic. The ER is often the first experience patients have with a health system, and we get pretty good (if not consistently successful) at service recovery. But service ethic goes beyond that. Hospital-based specialties (like radiologists, hospitalists, and pathologists) know that their hospital is their home, and part of how you take care of your home is participating – committees, fundraising, service, volunteering, and just leaning in. (I learned that from people like Greg Henry and Steve Holtzclaw, MD, MBA). There is always something you can do to help and eventually, it gets noticed. I know many people think that burnout is the main reason emergency providers end up in other careers. It’s true that emergency medicine is hard work, but you can’t really ‘burn out’ into a leadership position—it’s more of an 'all-in.' You have to love what you do and want to make it all better. #HospitalWeek #EmergencyMedicine
