Exploring Community Health Worker Perspectives: Maternal and Child Health Care Implementation in Rwanda. Uncovering Barriers and Facilitators Through Qualitative Inquiry.
This study explores the experiences of community health workers (CHWs) delivering maternal and child health (MCH) services in Rwanda. Through interviews and focus groups with 24 CHWs and supervisors, key facilitators like community trust and personal motivation were identified, while barriers such as long hours, limited resources, and low salaries were also highlighted. Understanding these factors can help improve MCH service delivery by addressing challenges and leveraging motivations among CHWs.

Community Health Promoters Embrace e-CHIS Across Kenya
Mary Muthoni, Principal Secretary for Public Health, has reported significant progress in the adoption of the electronic Community Health Information System (e-CHIS) for routine community health service delivery in Kenya. Currently, 46 out of 47 counties have integrated e-CHIS into their health services, marking a substantial advancement in health information management and service delivery efficiency. The only county yet to implement this system is Mandera, indicating a near-universal uptake of digital health solutions aimed at enhancing the quality and accessibility of community health services across the nation.

They live and work within the communities they serve. In one of the poorest and least developed areas in Kenya, I met one who mentioned that she walks around with batteries and razor blades.
She more often than not meets pregnant women on the road as they have to cover distances of more than twenty kilometres to access a health facility.

Community engagement is critical for democracy.
I am the only person from the U.S. in the course I'm taking this week (Working with Conflict and High Emotion, hosted by IAP2 Canada), and it has been fascinating to see how community engagement is influenced by culture.
Culture influences the degree to which the people's voice is valued. The World Health Organization is actively working to engage other countries, such as Thailand, to promote social participation (video).
I know many agencies in the U.S. government engage the public in decisions, but do we participate enough? Is public participation a large enough part of the process, and/or do we as citizens participate as much as we could?
And I'll just leave you with this to chew on...

Community consultations engaging the community in improving health services
This video is part of a series of audiovisual success stories depicting N’weti's projects and interventions. With this content we share N’weti’s interventions in the context of community-led monitoring of the quality of primary health care service provision using the Community Score Card (CSC) tool. The #CSC is a tool that allows users of public health services to participate, in an active and organized way, in monitoring the quality of services provided by health facilities. The #CSC, from N’weti’s perspective, is used for three main purposes: (i) monitoring the quality of public health services; (ii) identifying and defining health priorities (from the user’s perspective) for subsequent integration into district health sector plans and the Economic and Social Plans and District Budgets (PESOD); and (iii) building a body of evidence for defining advocacy actions on public policies in the health sector. The #CSC is a tool developed by The World Bank and adapted by N’weti for community-led monitoring of the quality of services. N’weti, in partnership with the Ministry of Health (#MISAU), implemented community consultation management processes using this tool from 2019 to 2023, in close collaboration with the targeted health units and local authorities, and at the time of producing the video (first half of 2023) community consultations had already been conducted in 273 health units in the provinces of Maputo, Gaza, Inhambane, Zambézia and Nampula

Main difficulties with community health digital solutions include limited access to technology, data security concerns, and a lack of alignment with patients' needs and values. Electrification of health facilities and access to electricity by CHWs are foundational for effective implementation of community digital health systems. They ensure reliable operation of digital tools and connectivity, essential for data collection, patient management, and telemedicine services, ultimately enhancing healthcare delivery. #CommunityHealth#DigitalHealth#HealthcareDelivery

Community health workers are the backbone of health systems around the world. I was glad to have the opportunity to recognise their crucial role by meeting with representatives from Ministries of Health from Burkina Faso, Cameroon, Nigeria and Uganda. We discussed their priorities and challenges in terms of the Community Health workforce and opportunities for empowerment through digital solutions and fair and reliable renumeration.
