SIMPLE BUT SHARP TOOL TO GET THINGS RIGHT It was a problem of global proportions demanding a global response. Around 2006, the World Health Organization (WHO) set out to improve surgical safety. Of 230 million surgeries performed worldwide (2004), at least seven million people a year were left disabled, and at least one million died...all due to avoidable human errors. An initiative by WHO led to a 2007 conference attended by doctors, anesthesiologists, nurses, and patients worldwide. On the table were several proposals, like training doctors and nurses, paying incentives for greater safety, or issuing guidelines. The ideas looked good, but none turned out to be practical or cost-effective. After exhausting all possible options, eventually, the panel came up with a solution that looked too simple to be effective: a surgical safety checklist. The checklist included seven checks before giving anesthesia, seven checks before incision, and finally, five more checks before wheeling out the patient from the operation theatre. Would it work? In a pilot study, the surgical safety checklist was implemented in eight hospitals around the globe. Over three months, the rate of major surgical complications fell by 36 percent, and deaths fell by 47 percent. This idea of using a checklist to avoid surgical errors was, however, borrowed from another industry: aviation. There is a reason why pilots are trained to strictly follow well-defined checklists at every stage of the flight. A simple but sharp tool, a checklist is a reliable way to overcome the limitations of the human mind that can forget things, take dangerous shortcuts, or get confused when confronted with complexity. Are education tables important? Yes, the circulating nurses and surgical techs are crucial to the success of surgical procedures! The OR staff does not always get the notice they need to know about new procedures or new set ups surgeons have been trained on and are looking to perform immediately. Be diligent in making sure you reach out to staff after surgeon trainings and have all the adequate pieces for education tables: demo product in all sizes, additional equipment or instrumentation, literature, studies upon request, sign in sheet, business cards & videos on demand. Make your demo short 1 minute or less and be ready to pivot to the tough questions as needed! This week on Promising Young Surgeon, I speak with Dr. Chris Kennel, a US born and trained ENT now practicing in New Zealand. We discuss the myriad of differences in hospital, training and surgical culture in the US vs the rest of the world. Since the COVID pandemic outbreak, there has been an influx of US trained doctors seeking relocation abroad, and New Zealand is a popular destination. It is easy for US based doctors to forget about the rest of the world and cling to the standards and practices set forth by our own system. But what if we consider that things can be done differently? Hearing from Dr. Kennel that tea, coffee and biscuits are commonplace in the operating theatre, that all physicians are represented by at least some form of union, and that trainees have strict duty hour protections in place to focus on learning nearly took my breath away. Imagine an operating schedule that prioritizes physician and staff wellbeing and results without compromising quality and outcomes. Well, it exists in New Zealand. I encourage everyone to give this episode a listen if only to expose yourself to alternative modes of thought.
