Sat through an interesting debate/disscussion at #THT2024 last week on why Left Ventricular Assist Devices (LVADs) may be under utilized. One surgeon suggested #medicaldevice companies need to consider marketing directly to consumers because most people don't know LVADs even exist. (She said one patient came to her asking to have an Impella implanted even though she wasn't undergoing surgery.) But another surgeon countered that LVADs won't be broadly adopted until they're less cumbersome to use, recalling that one patient told him all the wires and apparatus made him feel like a refrigerator None of that has anything directly to do with Sean Whooley's article in MassDevice. Here are Sean's details - "The company reported eight injuries and zero deaths related to this recall. In total, the recall affects 1,560 devices in the U.S. distributed between May 7, 2020, and Dec. 18, 2023. Abbott initiated the recall on Jan. 3, 2024." https://lnkd.in/erW7JuvA

This patient's iliac vein was being compressed and pinched in two areas, as shown in the before image. A compressed iliac vein is a serious vascular condition known as May-Thurner Syndrome. This condition restricts blood circulation from the leg to the heart, causing debilitating leg swelling, leg cramps, and leg pain. To open the iliac vein our vascular surgeons used advanced endovascular techniques, including the placement of a stent and the deployment of a balloon within the iliac vein. How do you prescribe preoperative exercise to your patients? The only intervention that fits all, almost.. Especially the motivated! Exercise has a positive impact on a wide spectrum of preoperative parameters, especially in vascular patients, with very little risk, but questionable applicability.. During my research, I was starting to notice a positive effect in preoperative patients who undergo a combination of resistance and light cardio training: frequently (i.e ever other day), and at an appropriate intensity (nowadays called zone 2 to 3 exercise). Carotid endarterectomy is considered the standard of care for people with moderate to high carotid stenosis. However, both medical therapy and surgical techniques have improved considerably since the landmark clinical trials were conducted 30 years ago.
