Epic Yale New Haven Health - Good progress. I'd still like you all to consider the whole person at once instead of just the presented symptoms, labeled condition, etc. as you said in your post "...Using Epic, clinicians can automatically generate care plans for patients with a specific issue, like COVID-19 or alcohol use disorder...." - These conditions, as well as all other "conditions" are multi-faceted having physical/emotional/psychological/environmental/personality/culture/etc. components which are all interconnected and create "health" or the lack thereof. Some day, I'd like you to think about holistic diagnosis and treatment looking at the "big picture" around symptoms/maladies. Perhaps the patient could enter a profile into an EHR (like Epic) and AI could provide a draft holistic personalized diagnosis/treatment analysis. Just an idea from a patient/healthcare consumer (me). When care delivery is inconsistent, it can lead to higher costs and worse clinical outcomes. Our friends at Yale New Haven Health are making significant progress addressing this issue by creating a standardized model for evaluating, diagnosing, and treating certain medical conditions. They call this their Care Signature Initiative. Using Epic, clinicians can automatically generate care plans for patients with a specific issue, like COVID-19 or alcohol use disorder. Since its launch in late 2019, Yale New Haven Health has successfully developed over 600 Care Signature Pathways. "We have gotten to a reduction in variation that has been a bedrock of quality improvement for years," says Chief Clinical Officer Thomas Balcezak, MD, MPH. Your US House Representative and Senators need to hear from you TODAY about how important #communityhealthcenters are and how many of their constituents rely on them for their care! Community Health Centers strive to be comprehensive medical, behavioral health, and dental homes for all patients. However, data from the Health Resources and Services Administration (HRSA) shows that health centers could serve their patients more holistically if they had additional staff and resources. Estimates show that 12.3 million more health center patients could benefit from behavioral health services, but shortages of behavioral health staff are what is severely limiting access.
