Communication breakdowns are the leading cause of sentinel events in #hospitals, so it’s no surprise that of the hundreds of challenges facing today’s #healthcare institutions, the leadership team at Carilion Giles Community Hospital (part of the Carilion Clinic hospital network) chose to prioritize improving handoff communications in their efforts to reduce patient harm. Over the past year, we’ve had the pleasure of helping nurses in CGCH’s ED and Med-Surg units implement a blueprint for structured handoff communication that allows transitions of care to occur reliably and uniformly while capturing often-missed patient information.
