Sports injuries are common, and the subsequent pain is a normal physiological accompaniment to such an injury. The experience of pain is shaped by neurophysiological, cognitive, affective, social, and environmental influences. Given these complexities and, often, the absence of clear clinical guidelines, pain management strategies for both the professional athlete and the “weekend warriors” vary and often do not reflect an understanding of the cause and type of pain experienced by athletes. Nonsteroidal anti-inflammatory drugs (NSAIDs) are the most frequently used pain medication. NSAID use rates are concerning for several reasons, including concurrent use of multiple NSAIDs, multiple routes of administration, higher than the manufacturer's recommended dosing, use for prevention of pain, limited evidence-based clinical guidance, adverse effects, and limited evidence regarding effects on injury healing. Existing empirical research does not provide a sufficient body of evidence to guide athletes and healthcare professionals in making analgesic medication treatment decisions. Surgery and anesthesia are critical health-care services that reduce the risk of death and disability among millions worldwide each year, and the need for these services is expected to continue to increase over the next decade. Globally, nearly 350 million operations were performed in 2022 whereas in the US an estimated 64 million inpatient surgical procedures and 25 million ambulatory surgeries were reported in 2022. Although possibly lifesaving, surgery is also associated with potential harm, which frequently includes pain during and after the procedure. Despite an improved understanding of pain mechanisms, increased awareness of the prevalence of postsurgical pain advances in pain-management approaches, and other focused initiatives aimed at improving pain-related outcomes in recent decades, inadequately controlled postoperative pain continues to be a widespread, unresolved healthcare problem. Suboptimal acute pain management in surgery patients is accompanied by an array of negative consequences, including increased morbidity, impaired physical function and quality of life, slowed recovery, prolonged opioid use during and after hospitalization, and increased cost of care. In addition, early postoperative pain appears to trigger persistent pain that may last for months after surgery in a substantial proportion of patients. To prevent the progression from acute to chronic postoperative pain, more aggressive analgesic/anesthetic measures are needed to reduce the incidence and intensity of acute pain during and immediately after surgery. There is increasing evidence of an association between the level of acute pain and the risk of developing chronic pain. This progression, for which the mechanisms are poorly understood, has been observed in various patient populations following surgical procedures.
