Island Harvest’s Community Empowerment Programs team was delighted to participate at Town of Hempstead Supervisor Don Clavin, the Hempstead Town Board, and the Department of Community Affairs’ Community Health Fair this past Saturday! The team was able to provide goodie bags of hygiene products while engaging in SNAP outreach for community members in the Hempstead area. At Operation Gateway, advocacy is at the core of all we do. We are staffed with Community Health Workers and one of the primary components of effective community health work is TRUST!!! How do you keep the flame of trust burning??? T R A N S P A R E N C Y Check out our year to date graphic. You will see how we use our resources. We have served 167 members and this is how the money spends!!! We are wide open with it because we love our community and we appreciate all of our partners. We do our best to connect with the case managers in the prison but they are short staffed and we work around by way of in reach process that OG COOP created in partnership with Brent Bailey at the Local Reentry Council and we also get community referrals from family members and other partners that provide services. We do what we have to do and CHWs are experts at figuring it out. IF YOU KNOW, YOU KNOW!!! We don’t just talk about “wraparound services,” we do this. Practical, tangible tactics!!! It’s only the beginning! #reentry #prisonreform #reformreentry #equity #access #impact #transparency #flyhighisaaccolemen #vayastrong #healthyopportunitiespilot #oghop #impacthealth #impactrecidivism #stigmaslayers #sdoh #minoritymentalhealthawarenessmonth #mentalhealthishealth

Of lately, #Kakuma is facing water crisis making people to go beyond limits in search for water. My Question is, What is the #International #Community doing in respond to this 🤔? It's an exciting week for BHPN as we launched the new Public Health Primer: Engaging Community Development for Health Equity! The primer was developed through a collaboration between Build Healthy Places Network and the National Association of Chronic Disease Directors (NACDD) to establish mutually supportive collaborations between these two sectors to advance health equity. It emphasizes the importance of resident leadership and provides guidance on how relationship-building can address injustices and promote equitable change in communities. 🌟 Community Health Improvement Week - June 1st Week 🌟 As we celebrate Community Health Improvement Week, let’s take a moment to recognize the incredible efforts of healthcare professionals, community leaders, and individuals who are dedicated to enhancing the health and well-being of our communities. This week is a reminder of the power of collaboration and the impact of our collective efforts. Focusing on preventive measures and promoting healthy lifestyles helps reduce the prevalence of chronic diseases. Improving access to healthcare services ensures that everyone receives the care they need, regardless of their background or circumstances. Educating the community about health risks and healthy habits empowers individuals to make informed decisions about their health. Building strong support systems within communities fosters resilience and improves mental health and well-being. Striving for health equity ensures that all community members have the opportunity to achieve their highest level of health. This week, let's celebrate the progress we've made and renew our commitment to building healthier, stronger communities. Whether through volunteering, advocating for better healthcare policies, or simply spreading awareness, every action counts. #CommunityHealthImprovementWeek #PublicHealth #HealthyCommunities #Wellness #HealthEquity #NAYS
