There have been notable changes in the requirements and usage of syringe drivers in healthcare settings since the COVID-19 pandemic. With increased demands on healthcare systems and changes in protocols, there have been adjustments in the use, maintenance, and protocols surrounding syringe drivers. New 2024 #ISMP guidelines spotlight RECURRING #medicationerrors, which, DESPITE former alerts, CONTINUE to occur, causing harmful and even fatal errors. YIKES. So, despite the Institute for Safe Medication Practices (ISMP) guidelines, the SAME errors continue to occur. ❓ Why does this continue to occur? ✅ System errors. Pure and simple. ❌ A guideline does not change culture. ❌ A guideline does not change the systems within a hospital organization. 🩹 Slapping a bandaid on a broken system will not solve the problem. Likely, it will create more. In order to affect REAL/LASTING change, IMHO, the CULTURE needs to be overhauled.
