A story on why clinicians need to know costs of their orders more than anyone. The acquisition cost of rectal indomethacin has gone up from $2 a dose to $325 a dose. The ASGE guidelines state there is a 50% reduction in post ERCP pancreatitis amongst all-comers undergoing ERCP making it a recommendation. It is unsustainable to use this medication on every patient undergoing an ERCP. What if I am merely exchanging a stent? What if the patient has had a sphincterotomy and I'm going to place a pancreatic stent? What if my rate of post ercp pancreatitis is low and I do 300+ ERCP's per year? Giving clinicians the cost data, the clinical synopses, and empowering them at the point-of-care is the essence of clinical-financial decision support. His results showed in a significant number of cases, specifically 42.7%, physicians decided not to treat the patient when home OCT data was present. 1- Clinicians should diagnose posterior semicircular canal BPPV when vertigo associated with torsional, upbeating nystagmus is provoked by the Dix-Hallpike maneuver. 2- Clinicians should not obtain radiographic imaging in a patient who meets diagnostic criteria for BPPV in the absence of additional signs and/or symptoms inconsistent with BPPV that warrant imaging. 3-Clinicians should not routinely treat BPPV with vestibular suppressant medications such as antihistamines and/or benzodiazepines. 4- Clinicians should use repositioning procedures or refer to a clinician who can treat, patients with posterior canal BPPV with a *canalith repositioning procedure*. #vertigo #dizziness #vestibularrehab #physiotherapy #BPPV
