Insulin detemir, sold under the brand name Levemir among others, is a long-acting modified form of medical insulin used to treat both type 1 and type 2 diabetes. It is used by injection under the skin. It is effective for up to 24 hours. Insulin detemir was approved for medical use in the European Union in June 2004, and in the United States in June 2005. It is on the World Health Organization's List of Essential Medicines. In 2020, it was the 124th most commonly prescribed medication in the United States, with more than 5 million prescriptions. It is an insulin analogue in which a fatty acid (myristic acid) is bound to the lysine amino acid at position B29. It is quickly absorbed after which it binds to albumin in the blood through its fatty acid at position B29. It then slowly dissociates from this complex. In India, there is a common fear that whey damages the kidneys. What many don't know is that pharmaceutical companies sell whey-based products for chronic kidney disease (CKD) patients, which are loaded with sugars in the form of maltodextrin and fructose. These products also list corn oil as the first ingredient. Corn oil has been proven detrimental to cardiovascular health, as highlighted in the Minnesota Coronary Experiment. (Ref: https://lnkd.in/d8NKSqNh ) This harmful formulation is given to CKD patients who often already suffer from diabetes and hypertension. Such products will only worsen their conditions, increasing the need for more insulin or blood pressure medications. When will we learn the correct nutrition for CKD patients? ⚠ - Small rant coming...just a heads up. I am reaching peak fatigue with people promoting their products as the equivalent to GLP1-receptor agonists. These drugs (semaglutide, tirzepatide, liraglutide, etc) bind to and activate the GLP-1 receptor in a manner similar to GLP-1 you make in your on body. This is like when we give estrogen to women in menopause or prednisone, a synthetic glucocorticoid that binds to cortisol receptors, to people with a serious inflammatory reaction. Compounds that stimulate the release of a hormone are not the same as receptor agonists. Lots of things can stimulate release of hormones, or even have hormone-like activity, but that does not per se make them effective. I am not saying that some products that increase GLP1 release are not helpful for metabolic health and weight management, but it's not the same mechanism and we should not anticipate the same effect. Consider that the primary things that stimulate the release of insulin are sugars, free fatty acids, and amino acids - but we would not equate these to replacing insulin in someone with diabetes.
