Trust..... it's critical in this partnership that we have agreed to. You have physicians referring new patients to your hospice. First phone call you make is to a transport service. Time is of the essence and you need a transport company that meets your time requirements - 7 days a week, 24 hours a day. You have 82 more calls to make and coordination that needs to be dealt with. DME, staff, Rx, and more. Why in the world is your transport company not the easiest one to deal with at all times? The partnership that we have agreed to also includes caring for your patients with the utmost respect, love and compassion. We talked about this vision and you can make it happen. A partnership with a transport company. One call and we take it from there. The partner you never have to worry about. We just get it done. In about a week’s time, Mobile Inpatient Care@Home (MIC) will become a part of the mainstream healthcare services. Over the past eight years, we’ve dedicated relentless effort to dismantle what we in Jaga-Me Mobile Medical & Home Care identified as the five walls of healthcare - physical, logistical, informational, financial, and cultural (trust) - with the greatest emphasis on overcoming the financial barrier. By bringing healthcare professionals directly to patients' homes, providing essential equipment and consumables, and fostering strong rapport with patient and family through comprehensive training, we've strived to break down these barriers and ensure access to care. Now, with this new financial model, patients and their families are empowered to select their preferred site of care without the looming concern of financial strain. This marks a significant dent in the formidable financial wall. Next up, we will work to scale this progress to ensure that quality home healthcare remains within reach for all. #HomeCare #HospitalAtHome #WorkInProgress https://lnkd.in/g_yHYEeX

NHS waiting list falls for third month in a row, thanks to efforts of staff The NHS waiting list fell in December for the third month running, while winter pressure continued to hit the health service hard with A&E and ambulance services experiencing their busiest ever January. England’s top doctor hailed the hard work and dedication of staff as new figures show the number of patients waiting for elective treatment fell by more than 19,000 in December, despite the NHS needing to reschedule 86,329 appointments due to industrial action. The new figures, published today, show that the overall Covid backlog fell by more than 6,200 in December for the third consecutive month and is now down by 164,898 since September to 7.6 million. Monthly performance statistics show that the proportion of people waiting over a year for elective care is the lowest it has been since November 2020, at 4.4% of the entire waiting list. NHS staff also delivered more elective activity in 2023 than in any other year since the start of the pandemic – with more than 17.3 million people treated. The latest data also shows the significant demand for services in January, as the NHS managed the longest period of industrial action in its history with 6 days of strikes by junior doctors. https://lnkd.in/eawrMtVB

Sr. Vice President at CARE COORDINATIONS. Connecting: Care Teams + Patients & Family + Referral Partners.
"..two types of statuses for hospital patients: “observational,” which generally requires a day or two in the hospital; and “inpatient,” which generally requires two or more overnight stays in the hospital. Right now, the patients that fall under the latter group are the only ones being admitted into CMS-sanctioned hospital-at-home programs." #healthcare #hospitalathome #homehealth #homecare

I love the shift from thinking about care in the home as post-hospital to instead be more patient centric - chronic condition management, longitudinal care. This is where we start to see the focus shift from preventing a hospital readmission to avoiding the need for a hospitalization. #careinthehome #patientcentric #populationhealth #chronicconditionmanagement #value
