#Safety_in_Aerospace_&_Defence. #I just watched the BBC TV Show #the_tragedy_of_Columbia released in February 2024. It’s about the tragic events that happened during the 2003 Columbia STS-107 mission that ended up with the orbiter disintegration when returning home. This documentary reveals what really went wrong with space shuttle Columbia and how crew and ship could have possibly been saved. After watching the TV serie, I remembered a lecture given by Admiral Harold W. Gehman Jr, the chairman of the Columbia Accident Investigation Board (CAIB) some years ago in one of my visits to Washington DC invited by USN PEOSUB. My understanding after Adm Gehman’s conference was that there are a lot of commonalities between submarine and aerospace communities. The USS Thresher tragedy (1963) resulted in an effort to prevent accidents: the #SUBSAFE program was initiated to identify critical changes to submarine certification requirements. After reading the Columbia Accident final report in full (my summer break :) I learned that the NASA “organizational culture” was in the spot at that time. The outcome is that the “certification team” needs to be totally independent from the “management team”, same as in the #SUBSAFE Program. A submarine and an orbiter follow the same principle: they both operate in the most demanding environments and conditions. I worked a lot about safety in submarines in my time as Submarine’s Certification Authority in the Spanish Navy and after reading the Columbia Accident Investigation report I reaffirm my point: in terms of “safety” we cannot afford to fail. I recommend to read this report and think about safety and organization’s culture.

On Thursday, the US Space Force successfully launched an operational weather satellite for the first time in a decade, marking a significant step forward in upgrading a meteorological network that has been in orbit for over half a century. The Ball Aerospace-built satellite was launched from Vandenberg Space Force Base on April 11 aboard a SpaceX Falcon 9 rocket, signaling the start of a modernization effort for the Space Force's 60-year-old weather constellation, the Defense Meteorological Satellite Program (DMSP).
