Working with senior hospital clinicians to help them prevent burnout so that they can rediscover balance and joy at work.
The Illusion of "Having It All" From an early age, many of us in the medical field bought into the idea that with enough hard work and sacrifice, we could eventually "have it all" - the prestigious career, a thriving family life, robust personal health, and time (& money!) for hobbies or travels. Yet, as we've navigated the intense realities of hospital medicine, that long-promised picture of perfectly balanced fulfilment has often felt like a mirage. The statement "you can have it all...just not all at the same time" rings painfully true. As doctors, we've had to make difficult compromises that once seemed unimaginable. Living apart from loved ones, especially for the many households with two medical professionals, has strained relationships. The inevitable tug-of-war between career demands and personal priorities has meant that family, social connections, or self-care have had to take a backseat at times. Ironically, the disappointment can feel most acute just when we think we've finally "arrived" at the epitome - after conquering brutal fellowship exams or achieving the coveted consultant position. Having climbed the professional mountain, the sobering realisation dawns that the "all" we've been chasing looks quite different than imagined. So what does "having it all" truly look like for those of us in hospital medicine? Perhaps it's about redefining priorities and letting go of static, idealised notions. It means being present and fulfilled in each moment, each season, whether that's immersing ourselves in developing medical mastery, nurturing family, or pursuing restorative hobbies. It's understanding that "all" is a constantly recalibrated balance. The path is nonlinear, the sacrifices are real, but therein lies the opportunity for purposeful choices aligned with our evolving values. Having it "all" may not be the rigid paradigm we once envisioned, but a mindful journey of making room for what matters most at that time.

I imagine that image might spark some controversy, but it’s not what you think. I love being a doctor. With a Fisher Price Medical Kit in hand, I was happy to dole out shots and take a blood pressure on anyone who would sit still by the age of 5. I researched the career in middle school and set out on a plan to make my dream a reality before I even stepped foot in high school. At that point in my life, I was motivated by my desire to become. Yet, as I settled into my career and physician identity, I started playing it safe. I didn’t have time to try new things because my career demanded my attention. I needed to keep working at a voracious pace to support my family. I didn’t have to try new things because my career path was set. I stopped trying to become the next version of myself because I feared what it might mean for my physician identity and career. It turned out that being a doctor was holding me back. Despite thriving in my career, I struggled to find fulfillment because I was no longer striving to create new goals and find new ways to ignite and explore my interests. The thing is, I didn’t even know it was happening. That’s the interesting thing about awareness. You don’t see it up until the moment that you do. I’m grateful for my physician coaches who helped open my eyes, raise my awareness, and cultivate curiosity and interest I didn’t know was hiding in the shadows. Being a doctor is no longer holding me back. Are you a physician who wants to transition into a non-clinical career, but your identity as a clinician is keeping you stuck? These 5 steps can help you loosen the grip of the physician identity. Step 1. Recognize how your physician identity resists change. The resistance might show up as procrastination, or tolerating a work situation that's no longer rewarding, or beating yourself up for not enjoying patient care anymore. Step 2. Appreciate how being a physician has served you. How have you benefited? What have you accomplished? Step 3. What new identity would you like to try on? Who do you want to become? What skills do you want to take with you? How do you want to live your life? Step 4. Take one small step and move into your new identity. Creating a new career identity takes time. It's not all or nothing: no matter what you choose to do next, part of you will always be a physician. Here's to your continued growth and evolution!
