Dear Nurse Intern [2] You can never know it all. Do not put too much pressure on yourself when you come across procedures you can’t carry out, questions you can’t answer, or complications you can’t manage. Remember, the one-year internship is an opportunity to improve at what you know and gain experience in areas you're unfamiliar with. Consistent practice breeds mastery. Don’t beat yourself up when you mess up a procedure the first time you try it; don’t shy away from it either. Do it scared until you’re able to do it like a pro. Don’t feel inadequate when you don’t have the answer to certain medical questions asked during your shift. Just ensure that the next time that same question arises, you have the right answer (always read up). Tip: Once you’re posted to a ward (e.g., renal ward), read up on major conditions treated in the ward, major medications used, and major procedures performed in that ward. Reading puts you a step ahead in terms of preparation. Ask questions when you’re confused. We deal with life, so never carry out a procedure you’re not 100% sure of. If you need to do it under supervision, please get someone to supervise (preferably a senior nurse). Always remember, "Repetition creates the master." - Cesar Millan. I read the article ”Emergency Medicine Paramedic” published by Mayo Clinic. Something I found interesting was that paramedics have a variety of responsibilities on the field. They are usually the first ones one scene with the patient. Paramedics are trained to handle many different types of situations.  In reading this article I learned a couple things. First, Paramedics are able to work in a couple of different places including ambulances, helicopters, hospitals, clinics, events etc. Second, It is very one-on-one patient care. You are with that patient until you reach the hospital. This means that you are able to put your full attention into helping these patients. Lastly, you never know where you’ll end up and what you’ll end up doing. This career is a very fast paced and changing environment. You could be assisting a patient who has fallen, or you may end up doing CPR on the way to the hospital.  I think that's what makes this job fun. Your environment is always changing, and you have to learn how to adapt. You get to help people in good and not so good times. Celebrating one year in my NP job 🎉 It's been a year full of learning and growth, and while the transition came with its challenges, it has been an incredibly rewarding journey.  Here are things I learned in my first year as an NP: 1. Imposter Syndrome is Real As a new NP, imposter syndrome can make you doubt your abilities, even when you're qualified. I was fortunate to have supportive colleagues and supervising physicians who helped me adjust to my first NP role. Their feedback on my work ethic and clinical skills reassured me during moments of doubt. Over time, these affirmations, combined with practical experience, helped me trust my skills. 2. Gain Enough Nursing Experience Before becoming an NP, I had 16 years of combined experience as an emergency nurse and nurse educator. This foundation was crucial in solidifying my assessment and diagnostic skills. I believe that having at least five years of nursing experience is essential before transitioning to the NP role. Those years help you manage complex cases and develop clinical judgment. My experience prepared me for a smoother transition and gave me the tools to handle a wide range of patient needs. 3. Maximize Your Clinical Internships NP education and clinical rotations are not standardized, and experiences can vary. Knowing this, I carefully selected my preceptors and clinical sites. I’m fortunate to have been mentored by experienced NPs and physicians who provided invaluable guidance. I took every opportunity to ask questions and stay engaged during my internships. This hands-on learning gave me the confidence I needed to transition smoothly into my NP career. 4. Work in a Supportive Environment I accepted a job in a busy practice with multiple providers on-site, a supervising physician down the hall, and an excellent team. This made my transition easier, as I could rely on my colleagues for support and guidance. Being in a collaborative environment where learning and mentorship are prioritized helped me build confidence quickly. The Qualified nursing faculty (MSN/PHD) in Pakistan is already insufficient, and students are lacking in clinical training and internships. Pakistan experiences significant changes in day and night schedules, with only about 8 hours of daylight during winter. Given that evening sessions for postgraduate programs occur after 1500 hours and sunset is at 1715 hours, it is impractical to conduct evening classes of daily 6 hours effectively. Postgraduate programs have 3-hour classes 2 or 3 days a week, but managing an undergraduate program with full 15/18 credit hours per week in the evening, along with clinical hospital settings, poses significant challenges. Doubling the number of nursing students will likely lead to a decline in graduate quality, increased faculty workload, and potentially exacerbate unemployment and low salaries for nursing professionals. The Higher Education Commission (HEC) should intervene, consider the implications of the PNMC's policies, and issue clear guidelines for offering evening sessions in nursing as well as other programs. #HEC #PNMC #BSN

🎈👏🎉 NURSES ARE EVIDENCE-DRIVEN. These are BSN students Madeleine, Abrar, Morgan, and Lauren who are wrapping up their third year of nursing school. Their evidence-based practice poster, "Pressure injury prevention in the ICU," earned a top spot overall at the School-wide poster session earlier this spring, and so earned a chance to attend UVA Health's Evidence-Based Project "day," which just took place May 6 and offered people across the health system a place a unit improvement ideas exchange (huge thanks to prof. Pam Baker DeGuzman, Kathryn Reid, and Meghan Mattos, who, with their teams, make this REAL). All BSN students take four semesters of R.E.A.L. across the course of their schooling, which offers Research, Ethics, Advocacy, and Leadership-infused learning. Working in small teams, they investigate a topic they've observed during clinical rotations and do a deep dive into the literature to find our more before figuring out how to present it to peers, professors, and colleagues. For some, the experience whets their appetite for more of this kind of work, which might drive them to do a summer research assistant internship, spur a Distinguished Majors project, and maybe begin to think about earning a doctoral degree. When nursing students get a sense of what research looks and feels like, and the plot points to doing an evidence-based research project, they're ready to use it in the environments where they ultimately work. The agency and know-how and POWER that understanding research processes allows truly sets UVA School of Nursing graduates apart. We are proud of their work and the voice and poise they develop as they dig in. During my internship, I met patients presenting with symptoms ranging from benign to life-threatening conditions. In the older adult population, the symptoms are often caused by the interaction or coexistence of multiple conditions/diagnoses, thus presenting unique clinical manifestations for each patient. The anamnesis is crucial in piecing the puzzle together to create an appropriate treatment plan. When a patient is incapacitated, family/caregiver input is pertinent in providing the necessary information. Older adults often depend on their caregivers due to their decreasing health and fitness, including their ability to access healthcare facilities. Based on my personal experience, it was not uncommon for this population to access healthcare not based on the severity of their condition but on the availability of their caregiver. The caregivers are often also busy with work, so they may not be aware of the full history of the illness nor the medications their elders routinely consume when asked by the healthcare professionals. Add the caregivers' obliviousness to the already complex pathophysiology within the patient, and we may end up with missing puzzle pieces of the patient's condition. It is why, during last Saturday's Posyandu ILP (an integrated primary care program run by the local Public Health Center together with health cadres focusing on preventative and promotive services) for the older adults cohort, I encouraged the attendants of the program to write information regarding their allergies, prior and current medical history, and the medications they have to take routinely on a piece of card fit inside their wallet. I asked them to do this with their caregiver so the caregiver also knows what information is on the card and where they put it in their wallet/purse. It would be even better if they checked the card with their primary care physicians. In previous research done on a different population, personal medical information cards (PMICs) containing contact and medical information helped provide the necessary information, speeding up interactions with professionals, and helping avoid repeat storytelling (https://lnkd.in/gU6T4wgU). Hopefully, by encouraging the older adult attendants to make similar cards can also help them if the situation ever presents itself. The session in which I was given the chance to speak during the posyandu ILP was part of RSIA Bunda Arif 19th-anniversary project in partnership with the local Public Health Center. Besides encouraging the making of PMICs, I also got to share a little bit about emergency symptoms that require a person's prompt admission to the ER.
