Daily Critical Care Pearls #172: Heparin-induced Thrombocytopenia - Part 2 📌 For a patient diagnosed with HIT who has just begun warfarin, you can give full dose anticoagulation with an appropriate agent and hold the warfarin and reverse it with vitamin K. 📌 For a patient with obesity, it is preferred to use argatroban as the chosen anticoagulant for managing HIT. Monitoring aPTT 2 or 3 hours after the rate change is appropriate based on the half-life, but consider extending this in patients with hepatic dysfunction to account for the longer half-life in such patients. #criticalcare #clinicalpharmacy #medicine #pearls

When it comes to hypernatraemia: The Systematized Nomenclature of Medicine Clinical Terminology February 2024 edition renamed diabetes insipidus as arginine vasopressin-related polyuria with two specific subtypes: arginine vasopressin deficiency and arginine vasopressin resistance. One of the reasons for this was shown in a recent survey of 1034 patients with central Diabetes Insipidus (CDI), 823 (80%) patients encountered a situation where a healthcare professional confused CDI with diabetes mellitus, and 884 (85%) patients supported renaming the disease https://lnkd.in/eQw5_kif #Nephrohealth #Nephrohealth_KA

Today is #WorldKidneyDay 💜 It is important for GPs to carry out simple blood and urine tests if a patient presents with potential risk factors for Chronic Kidney Disease (CKD). Making patients aware of the risk factors for developing CKD is crucial, as it often shows no signs or symptoms. Working in community pharmacy, you can also help to flag these risk factors if you have patients who come to you for medication for other conditions such as diabetes, high blood pressure or heart disease. Check out Irish Kidney Association at https://lnkd.in/eKBccK5g for more info. #WorldKidneyDay #KidneyDisease #ChronicKidneyDisease #CKD #Pharmacy #CommunityPharmacy #AHealthierHappierYou #MakingPharmacyEasier
