Today Reform Think Tank have published the first of several papers in our 'Reimagining Health' workstream, which seeks to promote radical reform of the health and care system. As covered in The Times today, our paper proposes devolving the vast majority of NHS budgets to regional mayors, and as a result, abolishing NHS England. The English health system is an international outlier in its centralisation, tightly managed from the centre despite rhetoric that stresses devolution. All the while, different demographic profiles, types of morbidity and health behaviours lead to highly divergent requirements for health services across the country. We therefore propose pooling and devolving the NHS budget to democratically elected regional governments, also allowing them to spend the budget on 'non-NHS' areas - such as housing, air quality or green spaces - where these improve overall population health. The latest Darzi review has been a great long-form capture of the current state of the NHS. Two things stood out to me- firstly that staff morale is so low and stress so high that in some areas, sickness related absences are as much as the equivalent of a whole month in year of work. The second is the recommendation to tilt towards technology- really to use technology to ease the burdens on the NHS. It is saddening and alarming to read the report, and the impact of the state of the NHS on its committed, talented but ultimately overwhelmed staff. Their disengagement is a very understandable response. Technology and automation can be a strong lever here- focusing on automating and streamlining aspects of the NHS that are routine, labour-intensive and boring, and don't require confidential patient data. This will free up time and bandwidth of many staff to focus on the judgemental 'human' aspects. This type of tech is easier to build and implement than clinical tools, which come with huge red tape. thanks for coming to my TED talk 🤓

The investigation explores the challenges facing the NHS, citing it as being in a “critical condition”. The recently published Darzi Report into the NHS offers a refreshingly honest view of the challenges the UK health system is facing. But do any of these issues sound familiar to anyone in New Zealand? • Underfunded primary care • Overburdened emergency departments • Confused management structures • Staff and patients feeling unheard In New Zealand, we’ve endured decades of underfunding, and now, as we consider further austerity measures, we must be wary of the risk of following in the UK’s footsteps. The NHS, after “the most austere period in its history with revenue prioritised over capital,” is seeing alarming outcomes— including deaths in emergency departments due to delays, the equivalent of an Airbus A320 crashing every week. If we don’t act soon, our healthcare system could face similar consequences. #Value innovation and delivery have to be based on #trust. To some extent #PCP may be trusted most because of the personalities and value systems that are drawn to this specialty. At the same time, assessment of trust by individuals presumably is driven mostly by #trustworthiness demonstrated empirically in an interpersonal #relationship over time that almost by definition is grounded in #shared understanding of lived experience. We need new business models and best practices that enable #healthcare to be continually #situated idiosyncratically in the place, pace, and purpose of a patient’s life. This potential extends far beyond what is formally recognized as healthcare and certainly beyond the institutional instantiations of sick care.
Amid declining trust in major institutions since the COVID-19 pandemic, most people still trust primary care clinicians for health info. Investing in comprehensive, community-based primary care can help rebuild trust in health care. It's time to act. https://lnkd.in/equA_Mcz

Hope Air released today the results of a survey of patients living in Northern Ontario. If there was ever any question that we need to do better in providing equitable access to health care in the North, this is what we found : - 1 in 5 residents living in Northern Ontario have had to cancel a medical appointment due to travel costs. This remains true for many residents even with existing partial support through the Northern Health Travel grant program; - 64% of residents in Northern Ontario had to travel outside their home community to reach health care - many travelling 9 hours or more each way; - 73% of residents in Northern Ontario believe that the Ontario government is responsible for funding equitable access to health care for all. We have a unique opportunity, right now, to ensure that everyone connects to the health care they need, no matter where they live, who they are, or what they earn.
