Centralized Nurse Resident Model: A Layered Approach to Supporting New Graduates Tracy Whitley, Novant Health, North Carolina I was most pleased with this programs decision to rotate the graduates through practice areas during the first year EVERY 4 MONTHS. This is a common practice in Australia and works well to give them a sense of what area of practice they most want to work in. The challenge is that if you don't keep them in one place long enough (at least through their first stage of transition which is about 4 months) they struggle with their confidence, skill development, cultural integration and stress management. 4 months should be the minimum before moving them again. In this program, if the resident is not 'fitting' where they are, the issues are identified, worked on, and if necessary the residents are moved either within that institution (grads are given alot of latitude as to where they might want to go - choice is about control +++) or within the region of institutions - so the health region does not 'lose' the graduate but is willing and able to be flexible with where they 'land'. I have noticed another theme in the programs - they communicate regularly, often, and with some structure with the grads. It's even better if this is congruent with the stages as the issues are somewhat predictable and when the grads 'hear' you ask questions that resonate so well with where they are at they feel understood and embraced. That's the goal! Another element of this program, which I need more details on, is 'graduation' - this is something we can ALL do - celebrate officially the 1-year graduation of new nurses.
