50 year old with left carotid occlusion and severe right carotid stenosis with a small stroke treated with carotid stenting via radial access. We have stopped doing pre plasty when using the wallstents. Saves time and steps and limits complications. One of those “keep it simple” cases where we dont even have to enter the aortic arch. Just straight ahead from the radial into the right carotid. 6F radial sheath, 6F Envoy, BMW wire, wall stent 7x40mm, NC emerge 5x20mm

Nice result. In fact, post-stenting angioplasty is associated with higher rates of periprocedural stroke and hemodynamic depression, possibly due to the "scissoring" effect through stent struts and plaque debris. Suboptimal pre-angioplasty seems to be better option.

I want to ask if you use web or not! And what about Distal embolic complication when you pass the stent without pre angioplasty
