Care transition remains risky for patients; what are we doing about it? That evening, I froze when I saw that the fresh blood on Mr X's leg was coming from under his robe. Mr X is an older citizen, a customer and friend, who particularly patronizes my pharmacy for a popular analgesic powder preparation, which contains high-dose aspirin and caffeine. I have persuaded him to no avail on cessation as he often jokingly retorted that he's probably been using the product before I was born. From his story: earlier that day, Mr X had had an elective surgery for a hydrocele removal. But unfortunately, his wound was still bleeding actively by nightfall, which was the reason he came to the pharmacy. I saw the problem immediately! I had no idea he'd got a surgery planned, and I was horrified to think he'd been taking his regular analgesic (aspirin is a potent anticoagulant!) all week while counting down to his surgery schedule. The surgical team most likely didn't obtain the history too. Bar any communication line to the facility of his procedure, I had to accompany Mr X back to the facility in person to give account of his medication history and suggest monitoring and prompt hemostatic interventions for him. Thankfully, Mr X is hale and hearty today. **** This is only a reference case of many illustrations of risk and hazards of care transition I and many pharmacists in Nigeria encounter to different degrees. Dangers in care transition is a global issue. But, in Nigeria with a prevalent problem of practice silos and inadequate shared knowledge of care among healthcare professionals, especially between community pharmacists and specialists, the risks of hazardous care transition for patients remains high! Yet this issue is typically unaddressed by policy and research. In our newest paper published in HSR-Wiley, I and colleagues, led this conversation discussing the realities and prospects of care transition performance of community pharmacies in Nigeria, from the angle of primary care where these premises are pivotal to health security in the nation. We highlighted the barriers and way-forwards to safe and effective transition of care from community pharmacies, hopefully for engagement by policymakers, researchers, and other stakeholders. Just published: In the realm of healthcare quality, outpatient parenteral antimicrobial therapy (OPAT) has emerged as the gold standard for managing patients who have transitioned from inpatient care but still require extended intravenous antimicrobial treatment. The adoption of OPAT at Al Hada Armed Forces Hospital in Taif, Saudi Arabia, not only bolsters patient satisfaction but also serves as a catalyst for reduced hospitalization durations, lower rates of emergency department readmissions, and an overall reduction in healthcare expenditures. Hyperkalemia, a potentially life-threatening condition characterized by high levels of potassium in the blood, is a common complication among patients with end-stage kidney disease (#ESKD). Given the critical role of the kidneys in regulating potassium levels, patients with ESKD are at a particularly high risk of developing hyperkalemia. This condition not only poses significant health risks but also contributes to a substantial economic burden on healthcare systems worldwide. Hyperkalemia in ESKD patients can lead to serious cardiac problems, including arrhythmias and cardiac arrest. The management of hyperkalemia typically involves acute and chronic treatment strategies to lower potassium levels, which may include dietary restrictions, medication, and in severe cases, emergency dialysis. These interventions, while necessary, contribute to the overall healthcare costs associated with ESKD. Direct Healthcare Costs The direct costs of managing hyperkalemia include hospital admissions, emergency room visits, medications, and dialysis treatments. Hospitalization rates for hyperkalemia have been on the rise, with associated costs significantly impacting healthcare budgets. Indirect Costs Beyond direct medical expenses, there are significant indirect costs associated with hyperkalemia in ESKD patients. These include lost productivity due to illness or hospitalization, long-term disability, and the need for caregiver support. Impact on Quality of Life The financial implications of managing hyperkalemia extend to the quality of life of ESKD patients. The stress of dealing with a chronic condition, coupled with the financial strain of ongoing medical care, can lead to decreased quality of life. Patients may face difficult decisions about their healthcare due to cost concerns, potentially leading to delayed or foregone treatments. Strategies to Mitigate Costs Efforts to mitigate the costs associated with hyperkalemia in ESKD patients focus on prevention, early detection, and cost-effective treatment options. Enhanced Patient Education: Empowering patients with knowledge about their condition and its management can lead to better self-care and potentially lower the need for acute interventions. Improved Monitoring: Regular monitoring of potassium levels by Alio can help in early detection and management of hyperkalemia, avoiding emergency situations that result in high costs. Cost-Effective Treatment Options: Research and development of more affordable medications and treatments can help reduce the financial burden on healthcare systems and patients. One of my colleagues in Abu Dhabi Health Services Company- SEHA - شركة أبوظبي للخدمات الصحية - صحة, Anas Salah recently published a study in a leading international, peer reviewed scientific journal, Clinical Nursing Research. Anas Salah The study investigates post-percutaneous coronary intervention chest pain (PPCP) among patients with coronary heart disease in Jordan. Chest pain affects 20 - 40% pof people at some point in their lives. The study revealed that chest pain (PPCP) affects a significant percentage of patients, with a decline in pain levels from admission to 30 days post-PCI. Predictors of PPCP include high-intensity physical activities duration per week, cardiac enzyme levels, increased ejection fraction, and higher heart rate. The study emphasizes the need for identifying PPCP predictors to target high-risk patients, potentially reducing readmission rates and improving patient outcomes. The prevalence and duration of PPCP in Jordan are highlighted, calling for further research to better understand this phenomenon. Here's the link:

Abdullahi of IPH, and Dr. Tasnim Dawoud and others from Shaikh khalifa Medical City Abu Dhabi Health Services Company- SEHA - شركة أبوظبي للخدمات الصحية - صحة examined the haemodynamic effects and incidence of hypotension associated with intravenous administration of acetaminophen among critically ill children, using a retrospective chart view. Findings suggest that the drug administration resulted in a haemodynamic event in 32% of the patients, whereas hypotension was observed in 24% of the patients. Mean arterial blood pressure was found to be significantly associated with both the outcomes. The study outlines the importance of exercising caution when administering acetaminophen to severely ill paediatric patients, as it may lead to clinically significant hypotensive changes.
