#Electronichealthrecords #ehrs are the backbone of modern healthcare delivery and vital to #healthcare #quality - but they are not without faults. Poorly designed, maintained, or implemented #EHRs complicate #clinician #workflows and add to the administrative burden. In extreme cases, they may even contribute to #safety issues including #diagnosticerrors. This risk is particularly acute for #raredisease #patients given the long diagnostic odyssey of 7 years or more many patients are facing, the limited familiarity of most healthcare providers with our diseases, and the many different specialties involve in caring for many of our patients. Interesting article in Health Affairs forefront today outlining concrete and practical policy solutions to address this issue, including the role various stakeholders including the Centers for Medicare & Medicaid Services #cms, the Office of the National Coordinator for Health Information Technology (ONC), the Joint Commission, and even FDA (as a model for data collection and event reporting) can play. No patient should be harmed just because an #EHR was not designed with the end-user in mind, obfuscates important clinical information, or is difficult to navigate and fit into clinical workflows.

Remote patient monitoring and Chronic Care Management go hand in hand. By partnering with RPM companies that provide ongoing monitoring of a patient’s vital statistics, our chronic care management team can be better equipped to manage the chronic conditions that, in the past, facilitated overuse and misuse of our emergency departments and acute care facilities. By addressing preventable escalations in medical conditions before they become emergent, we decrease the financial burden on the healthcare system. As Harris CCM and Esrun Health continue to grow and expand we look forward to providing every possible opportunity to service our patients and provide quality, cost-effective care. While CCM provides regular patient-provider interactions and 24/7 access to a healthcare professional, RPM takes this further by continuously monitoring a patient’s health status through technology and in-home devices, providing real-time data. This provides more opportunities for care plan adjustment to improve the management of patient conditions. By utilizing both CCM and RPM services – we can close care gaps in healthcare. Between the 24/7 access to a healthcare professional and the 24/7 health data monitoring provided by RPM, we can offer as much monitoring as any patient needs to avoid unnecessary hospital and emergency care services, thus decreasing the financial burden on the Medicare system. By providing these synergistic services to patients, we address: · Patient adherence and ability to understand medical advice, increase patient contribution to medical decision making, and better compliance with prescribed medications. · Caregiver burden – ability to support patient and maintain personal responsibilities and well-being, maintain societal role, and maintain own’s job. · Quality, safety, and cost-effectiveness of care by avoiding complications, duplicative visits and testing, poly-pharm situations, inappropriate care, emergency care misuse, and long waits for service. Common benefits of RPM include: o Continuous monitoring of patients to provide prompt care. o Improvement of patient self-care. o Efficient and effective communication. o Increased patient confidence. o Visualization of health trends. o Greater patient education. https://lnkd.in/gkZH49-x #wellnesswednesday #harrisccm #esrunhealth #weareharris #ccm #rpm #chroniccaremanagement #remotenurses

A recent Tebra survey revealed that 85% of independent providers believe that an increase in patient interaction would enhance health outcomes. Yet, many spend half as much time on EHR documentation as they are with patients, with 82% reporting charting and documentation as the #1 most time-consuming administrative task. Streamlining admin tasks and optimizing EHR systems is essential for improving care quality. At Tebra, we're dedicated to helping independent healthcare providers reclaim valuable time with patients and focus on what truly matters: patient health. 𝗛𝗼𝘄 𝗰𝗮𝗻 𝘄𝗲 𝗺𝗮𝗸𝗲 𝘁𝗵𝗲 𝗽𝗮𝘁𝗵 𝗺𝗼𝗿𝗲 𝗰𝗹𝗲𝗮𝗿 𝗳𝗼𝗿 𝗵𝗲𝗮𝗹𝘁𝗵𝗰𝗮𝗿𝗲 𝗽𝗿𝗼𝘃𝗶𝗱𝗲𝗿𝘀? In healthcare, making the right decision is like navigating a complex road. As Yogi Berra said, "When you come to a fork in the road, take it." But without the right information, even the best providers can find themselves stuck, unable to move forward. Those important decisions in healthcare can be really hard when doctors and nurses don't have all the information they need. Here are four ways we can get information to medical providers and improve patient care: 𝗨𝘀𝗲 𝗕𝗲𝘁𝘁𝗲𝗿 𝗧𝗲𝗰𝗵𝗻𝗼𝗹𝗼𝗴𝘆💻: Make sure doctors and nurses have fast and easy-to-use tools to find the information they need. 𝗢𝗿𝗴𝗮𝗻𝗶𝘇𝗲 𝗗𝗮𝘁𝗮 𝗖𝗹𝗲𝗮𝗿𝗹𝘆🗂️: Arrange patient information in a way that is easy to understand and quick to access. 𝗘𝗻𝘀𝘂𝗿𝗲 𝗧𝗶𝗺𝗲𝗹𝘆 𝗜𝗻𝗳𝗼𝗿𝗺𝗮𝘁𝗶𝗼𝗻⏰: Give medical providers the most up-to-date information as soon as it is available. 𝗥𝗲𝗱𝘂𝗰𝗲 𝗖𝗼𝗴𝗻𝗶𝘁𝗶𝘃𝗲 𝗟𝗼𝗮𝗱 ✂️: Cut down on unnecessary tasks so providers can focus on patient care. By doing these things, we can help healthcare providers make better decisions faster, leading to better care for patients and reducing the risk of provider burnout. 𝗛𝗼𝘄 𝘄𝗶𝗹𝗹 𝘁𝗵𝗶𝘀 𝗲𝗻𝗵𝗮𝗰𝗲 𝗽𝗮𝘁𝗶𝗲𝗻𝘁 𝗼𝘂𝘁𝗰𝗼𝗺𝗲𝘀? 𝗙𝗮𝘀𝘁𝗲𝗿 𝗗𝗶𝗮𝗴𝗻𝗼𝘀𝗲𝘀🩺: With quick access to complete patient histories, providers can diagnose illnesses more rapidly, leading to earlier treatments and better recovery rates. 𝗕𝗲𝘁𝘁𝗲𝗿 𝗠𝗲𝗱𝗶𝗰𝗮𝘁𝗶𝗼𝗻 𝗠𝗮𝗻𝗮𝗴𝗲𝗺𝗲𝗻𝘁💊: Real-time data helps providers avoid prescribing errors by checking for drug interactions and allergies, ensuring patients receive the safest and most effective medications. 𝗜𝗺𝗽𝗿𝗼𝘃𝗲𝗱 𝗖𝗵𝗿𝗼𝗻𝗶𝗰 𝗗𝗶𝘀𝗲𝗮𝘀𝗲 𝗠𝗮𝗻𝗮𝗴𝗲𝗺𝗲𝗻𝘁📈: Accurate and updated information allows for better monitoring and management of chronic conditions like diabetes and hypertension, leading to fewer complications and hospital visits. Providers need accurate and timely information to make the right choices for their patients. Sometimes, not having clear directions or data can cause confusion and waste time. This can impact patient care and make the job even more stressful for healthcare providers, leading to burnout. By leveraging advanced tools like MEDITECH #Expanse search and summarization, powered by Google Health, we help them choose the right path, leading to better patient outcomes and reducing burnout. #HealthcareInnovation #PatientCare #ProviderBurnout #MedicalTechnology
