Remote patient monitoring and Chronic Care Management go hand in hand. By partnering with RPM companies that provide ongoing monitoring of a patient’s vital statistics, our chronic care management team can be better equipped to manage the chronic conditions that, in the past, facilitated overuse and misuse of our emergency departments and acute care facilities. By addressing preventable escalations in medical conditions before they become emergent, we decrease the financial burden on the healthcare system. As Harris CCM and Esrun Health continue to grow and expand we look forward to providing every possible opportunity to service our patients and provide quality, cost-effective care. While CCM provides regular patient-provider interactions and 24/7 access to a healthcare professional, RPM takes this further by continuously monitoring a patient’s health status through technology and in-home devices, providing real-time data. This provides more opportunities for care plan adjustment to improve the management of patient conditions. By utilizing both CCM and RPM services – we can close care gaps in healthcare. Between the 24/7 access to a healthcare professional and the 24/7 health data monitoring provided by RPM, we can offer as much monitoring as any patient needs to avoid unnecessary hospital and emergency care services, thus decreasing the financial burden on the Medicare system.
